Objective: To explore the significance of serum vascular endothelial growth factor(VEGF) levels in children with hand foot and mouth disease(HFMD) complicated with encephalitis.To investigate the association between single nucleotide polymorphism of VEGF and HFMD complicated with encephalitis.Methods: This study research 236 HFMD patients in hospital,according to the clinical manifestations,they are divided into simple HFMD group inciude 145 examples and HFMD complicated with encephalitis group include 91 cases. This research approved by Soochow University affiliated children’s hospital ethics committee, specimen collection was asked consent of parents. All cases admitted to hospital were taked 2 ml peripheral blood, centrifugal into serum and blood cells on the first day.(1)Serum levels of VEGF were measured using ELISA in 30 children with simple HFMD, 45 children with HFMD complicated with encephalitis, 20 randomly selected cases of two weeks after the treatment from the children with HFMD complicated with encephalitis and 20 healthy children. Multiple factors logistic regression analysis was performed to analyze main risk factors in disease progression for HFMD complicated with encephalitis.(2)Genotype frequency and gene frequency of VEGF polymorphisms were performed by taqman probe method in 145 children with simple HFMD and 91 children with HFMD complicated with encephalitis.Results:(1)Serum levels of VEGF in children with HFMD complicated with encephalitis were significantly higher than in the simple HFMD group and control group(P<0.05), Significantly decreased serum levels of VEGF were found in the encephalitis group after treatment(P<0.05).(2) Serum levels of VEGF as an aid index in the diagnosis of HFMD complicated with encephalitis had high accuracy and strong Specificity. The statistically significant factors for prediction of disease progression from ordinary to Severe were duration of fever days, limb shaking, poor spirit, high blood pressure, fast heart rate, hyperglycemia, Serum levels of VEGF and EV71 infection(P<0.05). Limb shaking was the main risk factor for disease progression.(3) In +405G/C sites the frequency of G、C allele in HFMD complicated with encephalitis were 43.96%、56.04% respectively,and were 53.45%、46.55% in the simple HFMD, the difference was statistically significant. In-406T/C sites the frequency of T、C allele in HFMD complicated with encephalitis were 78.57%、21.43% respectively,and were 70.34%、29.66% in the simple HFMD, the difference was also statistically significant. But in +405G/C sites and-406T/C sites the frequency of genotype in HFMD complicated with encephalitis and simple HFMD was not statistically significant in comparison(P>0.05). In +936T/C sites the frequency of genotype and allele in the two groups was both not statistically significant(P>0.05).Conclusion:(1) Serum levels of VEGF as an aid index in the diagnosis of HFMD complicated with encephalitis had high accuracy and strong Specificity.(2) VEGF +405C allele、-460 T allele may be the susceptive gene of HFMD complicated with encephalitis.(3) To interpose and block VEGF may probably to be a new target spot to prevent the process of HFMD complicated with encephalitis.
