Diagnosis And Treatment Of Minimal Hepatic Encephalopathy With Critical Flicker Frequency In Patients Of Acute-on-chronic Liver

Objective:To study the diagnostic value of CFF (critical flicker frequency CFF) in MHE (Minimal Hepatic Encephalopathy) in ACLF (acute-on-chronic liver failure).Then to evaluate the influence factors of CFF through Logistic muitivariate analysis.Methods:Total of 90 patients with ACLF were enrolled in this study, who were without overt hepatic encephalopathy (OHE) underwent both methods at the same occasion, the critical flicker frequency (CFF) and Psychometric Hepatic Encephalopathy Score (PHES) for named Patients with Minimal Hepatic Encephalopathy. At the same time, set up the normal control group and patients with cirrhosis of liver control group.Those patients were provide some basic information and check blood ammonia concentration. Then patients who had the MHE would be injected Ornithine aspartate injection about lOg per day for 7 days. Then venous ammonia concentration, CFF and PHES. Were tested again.Results:Both tests were normal in 56.7%in the patients and both tests were abnormal in 36.7%in the patients. About more than 2/3 patients the two tests were in agreement as CFF classified 38.9%as PHES scale classified 41.1% of the patients as having MHE. The two methods were good linearly correlated (P> 0.05). Muitivariate analysis showed that age growing is ainfluence factor in the normal control group and age growing and blood ammonia level are the influence factors in ACLF with MHE.But gender, cultural degree, causes of liver disease, and others liver function level were not significantly different between MHE patients and without MHE patients.Conclusion:MHE incidence is higher in ACLF. CFF has good diagnosis effect in Identifying of ACLF with MHE compared with PHES.Logistic muitivariate analysis showed age growing is a influence factor for CFF; CFF can be usedto monitor of ACLF with MHE.
