Objective This research took Khitan descendants in Yunnan Baoshan as the object of the study. To understand HBV infection present situation of Khitan descendants in Yunnan Baoshan. To explore the relationship between HBV infection and living habits of the crowd. To study the correlation between the HLA-DRB1 genetic polymorphism and HBV infection of Khitan descendants in Yunnan Baoshan.Methods (1) The 325 subjects were enrolled from Khitan descendants in Baoshan. Unified questionnaires were distributed to them to investigate their life habits and customs. Collected venous blood 5ml and tested the Hepatitis B serologic indicators.x2 test and Logistic regression analysis were used to analysis the relationship between living habits and HBV infection of Khitan descendants.(2)The ELISA serological results of serum markers of Khitan descendants were divided into two groups, namely healthy control group of Khitan Descendants(n=70) and HBV infection of Khitan Descendants(n=10). Collected blood samples of healthy population and HBV infection people of Khitan non-descendants (40 cases each) at the same time. Extractedits DNA in whole blood, application-specific primers polymerase chain reaction method (PCR-SSP) detection HLA-DRB1 alleles, agarose gel electrophoresis showed amplification results. Using SPSS 19.0 statistical software, between the four groups each allele distribution differences between the two groups using the Chi-square test or Fisher exact probability calculation method for comparison. This study was to assess the association of HLA-DRB1 gene with HBV infection of Khitan descendants.Results (1) Epidemiology investigation result:10 cases were HBsAg positive in 325 Khitan descendants.The positive rate was 3.07%. All the HBsAg positive people were male. There were statistically significant differences among gender, family income, share a toothbrush, razor with respect to whether infected by HBV (p<0.05). Multivariate analysis showed that, history of hepatitis B vaccination as a protective factor (p<0.05). (2) HLA-DRB1 gene position examination results:HLA-DRB1*15 and HLA-DRB1*07 gene frequency in the healthy control group of Khitan descendants were higher, they were 15.71% and 12.14%; while the gene frequency of Khitan HLA-DRB1*07 and HLA-DRB1*12 were higher in HBV infection group of Khitan descendants, were 15%. A comparison of allele frequency was found between two groups, the differences in HLA-DRB1*15 between the two groups was significant, with statistical significance (p<0.05). In non-descendants of Khitan healthy control group DRB1*12 and DRB1*09 genes frequency were higher, were 15% and 6.25%, respectively; rather than in the group of HBV infections of Khitan descendants, the frequency of gene DRB1*03 and DRB1*04 were 13.75% and 7.5%. Allele frequencies were compared between the two groups, it was showed that the frequency of gene DRB1*03 and DRB1*12 were significantly different between the two groups, with statistical significance (p<0.05). Compared with the control group of DRB1 alleles in the healthy control groups and non-descendants healthy groups of Khitan, DRB1*07, DRB1*12, DRB1*15, and DRB3*01 alleles gene frequency were significantly different, with statistical significance (p<0.05). HLA-DRB1*12 allele in the HBV infection group of Khitan descendants compared with those in non-descendants of Khitan HBV infection had a statistically significant difference (p<0.05).Conclusions (1)Epidemiological investigation results showed:The levels of HBV infection in Baoshan Khitan descendants was low, the rate of infection was 3.07%. Hepatitis B vaccine was the protective factors. Male, sharing a toothbrush, razor were as their potential risk factors for HBV infection in Baoshan Khitan descendant population. Further investigations were needed. (2) HLA-DRB 1 genetic point test results showed:The HLA-DRB1*15 gene was the unique existence genes e had certain resistance effect of HBV infection possibly. HLA-DRB 1*12 could be susceptible gene to HBV infection of Khitan descendant population. As the result of the frequency of HLA-DRB 1*07 allele was higher, the Khitan descendant population HLA-DRB 1 gene distribution might have its unique characteristics. Compared with other nationalities’gene distribution in Yunnan province, there were certain differences. The HLA-DRB1*12 allele might be HBV resistance gene of Yunnan Baoshan non-Khitan populations, while DRB1*03 may be a susceptible gene for this group people.
