Objective: To investigate the knowledge about upper gastrointestinal cancer among high risk group in Urumqi, The time for the project is from June 2014 to January 2015,To understand the awareness of cancer knowledge cognition, the risk factors of upper gastrointestinal cancer, the willing of screening and paying among high risk group, To analysis factors that influences decisions and obstacles to being screened, and then make some suggestions on appropriate health education for community people. Methods:Selecting people who 40~69 years old answered questionnaire face to face in the new urban, The Shuimogou area, The Toutunhe area, The Midong area in Urumqi. The survey included the relevant cancer knowledge cognition. To analysis factors that influences the awareness of cancer prevention knowledge by ordinal-Logistic regression, To analysis factors that the compliance of screening on upper gastrointestinal cancer by Logistic regression. Results:(1)1150 people answered questionnaire in high risk group, 508 men and 642 women, The mean standard deviation age of people was 53.31±7.57.(2)772people had the results of endoscopic biopsy in high risk group, The detection rate of precancerous lesions is 17.1% among upper digestive tract cancer screening group.(3)The average of cancer knowledge cognition is 11, the highest score is 21.The awareness of early stages of cancer is 11%. The awareness of cancer knowledge cognition in people40 ~ 49 years old and 50 ~ 49 years old are higher than the awareness of cancer knowledge cognition in people 60~69 years old, OR=2.312, OR=1.766.The awareness of cancer knowledge cognition in participates who had greater than high school education is higher than participates who had high school education, participates who had junior high school education, participates who had less than Primary school, OR=0.511,OR=0.296,OR=0.144.The awareness of cancer knowledge cognition in participates who had family of history cancer is higher than participates without family of history cancer,OR=1.461.Having family of history cancer maybe a protective factor for the awareness ofcancer knowledge cognition.(4)The willingness of medical examination once a year is74.4%, 19.2% of people refused the paying of screening.(5)The compliance of screening in having diseases of upper gastrointestinal tract group was higher than without diseases of upper gastrointestinal tract group, OR=1.398.People who have the willingness of paying0 ~ 50 Yuan has the highest compliance of screening, OR=2.186.The compliance of screening in people who feel pain and discomfort of screening was lower than people who did not feel pain and discomfort of screening,OR=0.417.The compliance of screening in people who did not have symptom and people though the screening not having enough benefit were low, OR=0.383,OR=0.566.Education,The classification of body mass index and the score groups of cancer knowledge cognition are not significant in the regression(P>0.05).Conclusions:(1)The detection rate of precancerous lesions is high, Follow-up and supervise the precancerous lesions of upper digestive tract cancer in high-risk person. To improve the early detection of upper digestive tract cancer.(2)The awareness of early stages of cancer is low among high risk group, It is focus on the person with older and lower education to appropriate health education.(3)The willingness of medical examination is high, but the willingness of paying is quite low. The compliance of screening in having diseases of upper gastrointestinal tract group is high, people who had diseases of upper gastrointestinal tract pay more attention on their health. increasing the reimbursement rates health care and reducing the paying of screening may improve screening compliance.(4)The mentality of high risk group may influence the behaviour of screening,Health education and the communication of medical personnel with high-risk individuals must be strengthened to improve screening compliance.
