Objective:To compare the clinical effect and negative effect of metformin and insulin in the treatment of Gestational Diabetes Mellitus, to assess the clinical effect and safety for treatment of Gestational Diabetes Mellitus using metformin.Methods:Taking "Gestational Diabetes Mellitus, Metformin, Insulin" acts as key word, which was searched by computer in the following electronic databases as Cochrane Library、EMbase、Ovid、PubMed、CBM、CNKI、WanFang Data from inceptin to January, 2014. There were searched and collected clinical randomized controltrial (RCT) about the treatment outcome of Gestational Diabetes Mellitus using metformin compared with insulin. The included literatures need extracted data, and evaluated the methodological quality. The literatures meeting the standard will be quality-evaluated according to the standard recommended in "Cochrane System Evalution Handbook 5.0.1" and Meta-analysed with Revoew Manager 5.1 software.Results:There included 7 randomized clinical trials involving 1562 participants (778 cases appliedmetformin and 784 cases applied insulin), the meta-analysis shows that it was differentbetween metformin and insulin, the metformin reduces maternal wei-ght gain, gestational age at birth is smaller, it increases in the incidence of preterm birth, pregnancy-induced hypertension incidence is smaller and lower incidence of neonatal hypoglycemia (P<0.05).Conclusion:In maternal and neonatal outcomes, metfrormin is comparable with insulin, even has advantages on some aspects. Furthermore, it possesses high security, and imposes no adverse effects on pregnant women and newborns.
