Study The Value Of Diverse HPV Detecting Methods On Diagnosis Of Cervical Leison

Objective:This study investigated the statistic value of the Cervista test, Hybrid Capture 2(HC-2) test,Care HPV test for detection of high-risk human papilloma virus(HR HPV) DNA in cervical cytology specimens, and provided scientific basis and instruction for the cervical cancer screening programme in Xinjiang. Methods:The population consists of two parts.Those who fits standards were recalled for colposcopic examination and biopsies.321 cases were collected in total.There were 151 cases in population 1,which were detected with HC-2 test+Cervista test.There were 170cases in population 2,and HC-2 test+CareHPV test were used for HPV infection diagnosis.SPSS 19.0 was utilized to conduct statistical analysis.Statistical significance was determined by a=0.05.Results:Population 1:HR HPV Positive rates were 57.6%、54.3% diagnosed by HC-2 test and Cervista test; The consistency between HC-2 test and Cervista was valued by Kappa=0.892 (P<0.001); to detect women with Cervical Intraepithelial Neoplasia(CIN)Ⅱ or worse lesions,the ROC curve of HC-2 test was 0.744 (95%CI: 0.664～0.824, P<0.001), Cervista test was 0.786 (95%CI:0.715-0.858, P<0.001); The diagnose of A9 class by Cervista test correlate with pathological interpretations (χ2=43.063, P<0.001); (2) Population 2:Positive rates were 15.3%,21.2% diagnosed by HC-2 test and CareHPV test; the Kappa value of HC-2 test and CareHPV test was 0.686, (P<0.001); to detect women with CINⅡ or worse lesions,the ROC curve of HC-2 test was 0.940 (95%CI:0.884-0.995, P<0.001), CareHPV test was 0.845 (95%CI: 0.752±0.938, P<0.001); (3)The positive results of HC-2 correlate with age in population 1 (χ2=3.9, P=0.048)、population 2 (χ2=16.015, P<0.001), and also correlate with martial status(χ2=37.243, P<0.001)<. education level (χ2=14.365, P=0.002)、occupation (x2=30.907, P<0.001) Conclusions:1.There were good consistency between HC-2 test and Cervista test, also between HC-2 test and CareHPV test; In the detection of CINII or worse lesions,Cervista test shows good sensitivity and better specificity than HC-2,and an interpretation of A9 subgroup shows good predicted value in our study which is conducted in Xinjiang; 2.HC-2 test demonstrates better sensitivity than specificity when detect CINII or worse lesions.3 CareHPV test was comparatively economical-efficient,and could be used in screening of developing country.
