Objectives:In this research, we investigated the levels of serum NO、 e NOS and ADMA in hypertension complicating acute myocardial infarction, acute myocardial infarction, hypertension only and control,further to analysis the possible pathogenesis of hypertension leading to coronary heart disease and provide theoretical guidance for clinical early treatment. Methods:A total of 210 cases were involved in this research, 60 cases with hypertension complicating acute myocardial infarction group, 50 cases with acute myocardial infarction, 50 cases with hypertension only and 50 healthy cases as control, and there are not significant difference in the age and gender. The levels of serum NO、e NOS and ADMA were detected at the time of being in hospital. The determination measures of serum NO、e NOS and ADMA are nitrate reduction method, ELISA and high pressure liquid chromatography analysis respectively. The data were analysed with SPSS18.0 statistical software and numerical value was signed with mean ±standard deviation. Results:1. Levels of serum NO in four groups as follows: hypertension complicating acute myocardial infarction group(50.28 ±10.78umol/L); acute myocardial infarction(62.20±14.03umol/L); hypertension group(60.13±9.65umol/L); control group(75.27±75.27umol/L). The levels of serum NO in hypertension complicating acute myocardial infarction group were obviously lower than hypertension group and acute myocardial infarction group, while hypertension group and acute myocardial infarction group were obviously lower than control group.2. Levels of serum e NOS in four groups as follows: hypertension complicating acute myocardial infarction group(40.61±20.21 U/m1); acute myocardial infarction(56.70±18.96U/m1); hypertension group(54.25±12.89U/m1); control group(68.54±24.13U/m1). The levels of serum NO in hypertension complicating acute myocardial infarction group were obviously lower than hypertension group and acute myocardial infarction group, while hypertension group and acute myocardial infarction group were obviously lower than control group.3. Levels of serum ADMA in four groups as follows: hypertension complicating acute myocardial infarction group(2.86 ±0.22umol/L);acute myocardial infarction(1.73 ± 0.16umol/L); hypertension group(1.53 ± 0.19umol/L); control group(0.47 ±0.16umol/L). The levels of serum ADMA in hypertension complicating acute myocardial infarction group were obviously higher than hypertension group and acute myocardial infarction group, while hypertension group and acute myocardial infarction group were obviously higher than control group.4. Levels of serum NO、e NOS、ADMA in the groups of hypertension 1 、 2 、 3 level respectively as follows: serum NO levels(67.16 ±11.48umol/L、56.87±16.03umol/L、44.02±16.82umol/L); serum e NOS levels(60.27±16.76 U/m1、49.43±19.22 U/m1、37.45±14.10 U/m1);serum ADMA levels(0.87±0.12umol/L、1.92±0.19umol/L、2.93±0.23umol/L). All of serum NO、e NOS、ADMA levels in the groups of hypertension 1、2、3 level have significant difference.5. Pearson correlation analysis showed that the levels of serum NO and e NOS were positively correlated(correlation index = 0.601); the levels of serum NO and ADMA, e NOS and ADMA were negatively correlated(correlation index =-0.396,-0.475); the levels of serum NO、e NOS and the grade of hypertension were negatively correlated(correlation index =-0.412,-0.236); the levels of serum ADMA and the grade of hypertension were positively correlated(correlation index = 0.717).6. Logistic multivariate regression analysis showed that NO 、ADMA、age、TC、diabetes and hypertension may be independent risk factors while gender、 alcohol、 e NOS and TG is not independent risk factor for acute myocardial infarction. Conclusions:1. NO and e NOS levels were significantly increased while ADMA levels were decreased significantly in hypertension and acute myocardial infarction patients.2. Hypertension leading to acute myocardial infarction may be associated with the change of NO、e NOS and ADMA levels.
