ObjectiveThe aim of this study was to ascertain the role of cardiovascular factors in the development of ischemic colitis,and was to compare the prognosis of ischemia isolated to the right side of the colon with those of ischemia involving other segments of the colon.MethodsTwo retrospective studies were undertaken.One retrospective study was undertaken of patients with colon ischemiahospitalized during the interval 1999-2010 in General Hospital of PLA. IC was diagnosed on the basis of endoscopic findings and histology.Controls were randomly chosen from patients who were admitted in the same period and colonoscopically, diagnosed as ulcerative colitis.A conditional logistic regression was performed.Another retrospective study was undertaken of patients with colon ischemia hospitalized during the interval 1999-2010 in General Hospital of PLA and Beijing Hospital.Patients were divided into two groups: those with isolated right colon ischemia (IRCI) and those with colon ischemia not involving the right colon in an isolated fashion (non-IRCI). Only patients with colonoscope procedure-proven ischemic colitis were entered into our study.ResultsThe following variables were independently correlated with the development of IC: diabetes mellitus [odds ratio (OR=1.6, P<0.001]; dyslipidemia (OR=2.44, P=0.024); heart failure(OR=2.28, P=0.026); peripheral arterial disease (OR=4.64, P=0.04).A total of 170 cases of colonoscope procedure-proven colitis were collected, of which 36(21.18%) were isolated to the right side. Of these IRCI cases,61.11% had an unfavorable outcome compared with 8.96% of cases of non-ICRI:50.00% of IRCI patients required surgery compared with 7.46% of non-IRCI patients; mortality in patients with IRCI was 16.67% compared with 1.49% in patients with non-IRCI. Conclusions1、The development of IC was independently correlated with diabetes, dyslipidemia, heart failure, peripheral arterial disease.2、A total of 170 cases of colonoscope procedure-proven ischemic colitis were identified of which 36 (21.18%) involved only the right side. Patients with IRCI had a worse outcome than those with colon ischemia involving other colon segments.
