The Establishment Of The Professional Groups Hypertension Prevention And Control Mode And The Intervention Effect Evaluation

Objective To develop primary prevention of cardiovascular disease and and management in patients with hypertension, to improve the control rate of hypertension in the crowd, to prevent cardiovascular and cerebrovascular diseases and to achieve experiences of the prevention and treatment of cardiovascular disease in occupation through the establishment of cardiovascular disease prevention management platform in the occupation place.Methods By using the method of epidemiological field experiment, we established the platform of employee cardiovascular disease comprehensive prevention and control cooperating with a large state-owned enterprises in our city, and taking advantage of enterprise hospital, union, logistics and other departments.Setting up a project organization Including the expert group and executive group. Expert group:cardiovascular experts from Beijing Fuwai Cardiovascular Hospital, the First Affiliated Hospital of Anhui Medical University and Anhui Cardiovascular Provincial. Executive group was mainly composed of trained person from enterprise department of hospitals, unions, logistics.Project training Before field work on the project site began, the project expert group trained up the specialist, participating doctors and project managers and other project implementation team. The training content included “the primary prevention of cardiovascular disease on workplace” and “the hypertension management program on workplace”.Implementation plan:(1)The primary prevention of cardiovascular disease on workplace: Based on the enterprise 1880 employees of physical examination, wediscovered the risk factors of major cardiovascular diseases on the occupation place. Health monitoring points was established to encourage regular measurement of blood pressure, weight, so we could detect timely the risk factors of cardiovascular disease.(2)The management program of hypertensive patient on workplace: Screening managed objects, and voluntary assigning agreements with patients, making individual management scheme, setting up follow-up visit management mechanism,and finally making use of information management platform we accomplished the 24 months management objectives.Results Before the implementation this project, we detected the enterprise was at a high prevalence of hypertension, most of which were primary hypertension, based on the enterprise 1880 employees of examination. What’s more, most of them presented the risk factors of high blood sugar, high cholesterol, obesity, smoking, high salt diet. After the implementation of prevention and control mode, the one year later examination results showed that the incidence of hypertension significantly reduced. And through the health management of 216 patients with essential hypertension,the control rate of hypertension increased significantly.Conclusions Implementing workplace chronic disease prevention mode is practicable and effective. It was essential to establish the management platform of comprehensive prevention of cardiovascular disease and carry out prevention work on the grassroots.
