Aims: Currently, as chronic heart failure(CHF) growing public, biomarkers are playing more important role in diagnosis, assessment and prognosis of CHF. To verify whether soluble glycoprotein 130(sgp130), heat shock protein 27(hsp27), dipeptidyl peptidase IV(dpp4) and cathepsin S(ctss) could be candidate biomarkers in diagnosis of CHF in patients with valvular heart diseases,we compared the plasma concentrations between CHF and normal cardiac function group(NCF).Methods: Based on the previous study of human pericardial fluid proteome, bioinformatics analysis was performed and the four proteins metioned above are finally chosen. All subjects underwent resting 12-lead electrocardiogram, routine blood test, renal function test, and comprehensive transthoracic Doppler echocardiography in our hospital. CHF(n=50) samples were collected after in-hospital stabilization and the NCF(n=50) samples were gathered from medical examination center of Nanjing first hospital. All the blood samples were and stored prior to assay in batches. Test and compare the plasma concentrations of sgp130, hsp27, dpp4, ctss and NT-pro-BNP from the two groups by Elisa, and analyze the data to evaluate the efficiency of the diagnosis of chronic heart failure caused by valvular heart diseases.Results: Chronic heart failure patients with valvular heart diseases showed significantly higher mean concentrations of sgp130(317.3ng/ml vs. 215.9ng/ml), hsp27(2601pg/ml vs. 923pg/ml) and NT-pro-BNP(982pg/ml vs. 332pg/ml) compared with normal cardiac function group adjusting for clinical covariates(p<0.05), but not dpp4(7081pg/ml vs. 6930pg/ml) and ctss(985pg/ml vs. 1050pg/ml).Receiver operating curve analyses was performed to test the abilities of sgp130, hsp27, and NT-pro-BNP, which showed that hsp27 has the largest area under the curve(AUC)(0.920) as an individual biomarker to differentiate CHF from NCF. Furthermore, the combination of sgp130, hsp27 and NT-pro-BNP had the largest(AUC) of 0.969 compared to three individual biomarkers alone and the efficiency of diagnosis was significantly improved. The levels of all the three individual biomarkers showed no statistical differences between NYHA class II and class III/IV by compared their concentrations respectively.Conclusion: In this study, we verified that sgp130, hsp27 and the combination of sgp130, hsp27 and NT-pro-BNP could be efficient biomarkers to diagnose CHF with valvular heart diseases in all NYHA classes and worth further investigation.
