Objective This study is designed to explore the clinical characteristics and prognosis of patients with post-traumatic myocardial infarction by case analysis and following-up of the patients admitted to Fuwai Hospital since 1999.Methods 8 patients were admitted to Fuwai Hospital after post-traumatic myocardial caused by blunt chest trauma since 1999. The in-patient case history were reviewed to analyze the causes, risk factors of coronary artery disease, coronary angiography results and cardiac function evaluation, and telephone follow-up, outpatient medical record and rehospitalization history were collected to analyze the major adverse cardiac events in order to evaluate the prognosis.Results Selected patients were male,7 cases are under 40 years old and the average age is 32.13±8.53,none of them belongs to the high-risk population of coronary artery disease. The causes are traffic accident injury, blunt hitting and hurt, which the traffic accident is major. The coronary artery lesions are mainly of single vessel(7 are single vessel affected,1 is normal),which the major regions are left anterior descending branch and left main coronary artery.6 of them show the cardiac dysfunction and the average EF value is 42.74±10.48%.3 cases of left ventricular aneurysm were discovered.4 of them accepted surgery including Coronary artery bypass grafting and left ventricular aneurysmectomy,l of them accepted the percutaneous coronary intervention and 3 chose the drug conservative treatment. With 0.5-6 years follow-up, there are 2 cases of major adverse cardiac events, including ejection dysfunction and ventricular aneurysm formation, and both of the 2 cases showed rapid formation of ventricular aneurysm and ejection dysfunction after myocardial infarction.Conclusions Post-traumatic myocardial infarction are more common in the youth without high-risk of coronary artery disease caused by blunt injury especially traffic accident, with normal coronary arteries or a single lesion in left anterior descending branch or left main coronary artery. Sometimes the ejection fraction may be affected. The prognosis of patients mainly depends on the cardiac output function after myocardial infarction, which ejection dysfunction and ventricular aneurysm formation are the most common major adverse cardiac events, which the early appearance and rapid progress are the signs of unfavorable prognosis.
