pjective:This study by investigating the quality of life of patients with nasopharyngeal carcinoma radiotherapy after discharge and the current situation of self-efficacy level, On patients with nasopharyngeal carcinoma radiotherapy quality of life after discharge correlation with self-efficac, Analysis of influence of nasopharyngeal carcinoma radiotherapy patients after discharge self-efficacy main influencing factors, In order to promote and improve the quality of life of patients with nasopharyngeal carcinoma radiotherapy after discharge to provide theoretical basis and guidance。Further for grouping the research object, Enhanced self-efficacy interventions, To keep its high level of self-efficacy, conducive to effective disease rehabilitation and strengthening self-management, so as to prevent, reduce and control the forward radiation toxicity, improving patient’s quality of life.MethodsDescriptive research:This study choose in January 2013 to April 2013 in major medical affiliated hospital of Guangxi branch hospital after radiotherapy of nasopharyngeal carcinoma radiotherapy patients 240 cases as the research object. Random application of general data questionnaire, self-efficacy scale, depression scale, quality of life table and positive coping styles scale respectively in 240 cases of nasopharyngeal carcinoma patients during discharge, discharge after 3 months,6 months after hospital discharge, and 1 year after discharge, the situation of the quality of life and self-efficacy, Analysis can collect all the factors, find the main factors influencing the nasopharyngeal carcinoma patients after discharge self-efficacy.Intervention study:Selection in May 2014 to August 2015 in affiliated hospital of Guangxi medical university hospital after radiotherapy of nasopharyngeal carcinoma patients,120 cases of radiation as the research object, According to the random sampling can be divided into experimental group 60 people, the control group 60 people. The control group using conventional nursing after discharge method health education, the experimental group on the basis of conventional nursing care and effective discharge self-efficacy reinforced interventions (according to the survey factors aiming at the problems in patients with health education, including life coaching, oral health guidance, functional exercise and the guidance of nasal irrigation, activity and rest arrangement, remind the patient follow-up time, etc.). Random application of general data questionnaire, self-efficacy scale, depression scale, quality of life table, positive coping style scale and radiotherapy adverse reaction scale respectively in different times after discharge (at discharge, discharge after 3 months,6 months after hospital discharge) two groups of patients self-efficacy level, quality of life level, the degree of depression, coping styles, according to the medical behavior and to evaluate patients after radiotherapy complications, Compare two groups of patients at different times of the degree of self-efficacy level score, depression score, sequelae after radiotherapy, according to the medical behavior, quality of life score index were analyzed.Using SPSS16.0 statistical software to establish database, data entry, sorting, statistical processing and analysis, using descriptive analysis (percentage, mean ±standard deviation), t test, analysis of variance and rank and inspection, chi-square test, correlation analysis and multiple stepwise regression analysis and other statistical method. Resultsresults of the questionnaire1. After radiotherapy of nasopharyngeal carcinoma patients after discharge self-efficacy level:survey of 240 patients at discharge, discharge after 3 months,6 months after hospital discharge, and one year after discharge of the level of self-efficacy was respectively (29.13±5.28 26.02±5.88 25.14±5.28 27.10±5.42, average (26.85±5.53), Only17.71% score index> 80%, at a higher level,82.29% at low levels.2. The influence factors of discharged patients with nasopharyngeal carcin oma (NPC) self-efficacy level analysis:①Single factor analysis:Nas opharyngeal carcinoma patients after discharge of the self-efficacy leve 1 and the degree of the patient’s own culture, economic situation, posit ive coping styles, according to the medical behavior and the quality o f life were positively correlated (p<0.01); and the degree of the pati ents with depression, and negatively correlated with negative coping st yles (p<0.01);Itself with the patient’s gender, age, marital status, and occupation (p>0.05)。②Multi-factor stepwise regression analysis:N asopharyngeal carcinoma patients after discharge of the self-efficacy le vel and degree of depression, negative coping style and active coping styles, economic conditions, cultural degree, according to the medical behavior and the quality of life has significant correlation (p<0.01), is one of the highest impact related to the degree of depression levels. ③Patients with nasopharyngeal carcinoma discharge factors (degree of depression and coping styles) compared with the norm:The degree of depression and negative coping styles were higher than norm (p< 0.01); positive coping style is lower than the norm (p<0.01), the diff erence was statistically significant.3. In patients with nasopharyngeal carcinoma after radiotherapy quality of life after discharge conditions:Patients’quality of life after discharge is out of the hospital, discharged from hospital after 3 months and 6 months after hospital discharge change is bigger.4. The quality of life of patients with nasopharyngeal carcinoma discharge self-efficacy of relevance:After radiotherapy of nasopharyngeal carcinoma after discharge the patient’s quality of life and self-efficacy levels were positively correlated (p<0.01), Self-efficacy level is higher, the higher the quality of life level, on the contrary, the lower the level of self-efficacy, the lower the quality of life. Two groups of patients with nasopharyngeal carcinoma after discharge compliance comparison:After intervention, the two groups of patients after discharge compliance with statistical significance (p<0.05), the experimental group compliance are higher than the control group. Conclusion1. The radiation and chemotherapy in patients with nasopharyngeal carcinoma (NPC) after discharge of the overall level of self-efficacy is low, the level of depression, negative coping style and active coping styles, and degree of economy, culture is the influencing factors of nasopharyngeal carcinoma patients after discharge self-efficacy level, levels of depression are the main influencing factors.2. Radiation and chemotherapy patients with nasopharyngeal carcinoma (NPC) as the discharge time advance their quality of life have a downward trend, the quality of life scores and the correlation between each dimension and self-efficacy.3. The radiation and chemotherapy in patients with nasopharyngeal carcinoma (NPC) after discharge for depression, anxiety and other negative emotions and reduced compliance and an increased incidence of adverse reaction, such as degree aggravating, the implementation of self-efficacy increased after the intervention could alleviate the adverse reaction of patients, alleviate the bad mood, improve the patients’rehabilitation exercise adherence, so as to improve the quality of life of patients.4.
