Comparison Of Ultrasonography And Magnetic Resonance Sialography In Diagnosing Sjogren’s Syndrome

Objective:By comparing ultrasonography with magnetic resonance sialography(MRS) in diagnosing Sjogren’s syndrome, the aim of research was to study the clinical role of these two methods, and further study the value of ultrasonography in diagnosing Sjogren’s syndrome(SS).Methods:Fifty-nine cases in SS group and forty-two cases in control group underwent ultrasonography and MRS examination on the parotid gland, and all parotid glands were graded according to the ultrasonic-structure. The peak systolic velocity and resistance index of the temporal shallow artery were observed by pulsed wave Doppler. At the same time, the bilateral duct of all parotid glands were rebuild and the biggest diameter of each ductal expansion was measured after MRS reestablishment, then all cases were graded according to the size of the diameter. The above results of the research were comprised with the clinical diagnostic criteria and labial gland biopsy at last.Results:The accuracy, sensitivity, and specificity for the diagnosis of ultrasonography were 87.1%,89.8%,83.3%; MRS 90.1%,93.2%,85.7%, and labial salivary gland biopsy 87.1%,86.4%,90.4%, respectively. ROC curve showed that there was no obviously significant difference in the diagnostic value among three methods. In fifty-nine cases of SS, forty-eight cases had the identical grading diagnosis by ultrasonography and MR sialography, and thirty-two cases in the control group. The grading distributions by ultrasonography and MR sialography were different, and the differences were statistically significant (P<0.05). Kappa analysis showed good consistency (Kappa=0.736) in grading diagnosis of the two ways. The spearman’s correlation for the two methods was also very close (r=0.882, p<0.001), and the ROC curve analysis showed there was no significant difference in diagnostic value between ultrasonography and MR sialography(P> 0.05). And there was no significant difference between PSV and RI in two groups.Conclusion:Ultrasonography, MRS and labial gland biopsy had the same diagnostic value on SS, and it has good consistency between ultrasonography and MR sialography in grading diagnosis on SS. Therefore, ultrasonography has high value in diagnosing the degree of involved parotid gland of SS, and it can be used for clinical evaluation on patients with SS.
