Objective: T2 DM and obesity are the serious non communicable diseases, and harmful to human health.T2 DM is an endocrine metabolic disease characterrized by hyperglycemia,Persistent hyperglycemia and long-term metabolic disorders can lead to systemic atherosclerosis,then cause the damage of tissues and organs, especially the cardiovascular damage.In recent years, the incidence of obesity is increasing year by year.Obesity has become an important disease together with T2 DM hazard to human health.A number of studies indicate that insulin resistance(Insulin resistance,IR) is a common pathological basis of obesity and T2 DM,and has become the focus of the University’s science research.With the research of adipokines and insulin resistance,a growing body of researches suggest that adipokines cytokines is involved in IR.RBP4 is a newly discovered adipokines.Serum RBP4 is mainly generated in hepatocytes,and plays an important role in the physiological function of retinol.With the further study of RBP4,more and more researches suggest that RBP4 is involved in the metabolism of sugar,and IR, thus leads to diabetes and obesity.This article detectes serum RBP4 levels of NGR-NW group, NGR-OW/OB group, NW-T2 DM group and OW/OBT2 DM group,and detectes the correlation between RBP4 and glucose, insulin resistance index and lipid metabolism.Methods:1 Source of the study subjectsSelect 68 cases of individual into the T2 DM group,69 cases of individual into the healthy control group from the patients in the second hospital of Hebei Medical University during the period of April to August in 2014.The T2 DM patients needed to meet the following conditions:①Meet the diagnostic criteria for diabetes formulated by the WHO in 1999;②All patients had not yet received the treatment of diet control, exercise, taking hypoglycemic drugs or injection of insulin.Divide all the study subjects into 4 groups according to the obesity diagnosis criteria of Asians set formulated by the International in 2000.33 cases in NGR-NW group,the average age was 48.85±2.69 years old;36 cases in NGR-OW/OB group,the average age was 54.17±2.32 years old;31 cases in NW-T2 DM group,the average age was 59.00±5.00 years old;37 cases in OW/OB-T2 DM group,he average age was 56.00±4.00 years old.2 Collection and Measurement of the general informationAll subjects were performed by a detailed physical examination. Measure their height、weight before hat, slippers. And BMI were calculated by the formula, BMI = weight(kg) divided by height(m) squared.Waist and hip were measured,waist length was equal to the level of the waist by the umbilical point,hip was equal to the circumference of greatest hip. Calculate the WHR by the formula of waist circumference/ hip circumference.All subjects were fasted for 10 to 12 hours. Fast vein blood in the morning,determine the FPG、FIN、TC、TG、HDL-C and LDL-C,use Shanghai RD company’s ELISA kits serum RBP4.3 Statistical data analysisUse SPSS 13.0 statistical software,test normality and homogeneity of variance.If the data meet normality, homogeneity of variance,data were expressed as( ? ±S),using analysis of variance.If the information did not meet the normality,data were expressed as(Median±QR),using analysis of rank.Use Pearson correlation analysis,calculat regression coefficient,and test the coefficient.Results: Divide all the study subjects into T2 DM group and NGR group,the serum RBP4 levels of T2 DM group was significantly higher than the NGR group([1.08±0.59]vs[0.80±0.64]umol/L,P=0.043).Divide all the study subjects into NW group and OW / OB group,the serum RBP4 levels of OW / OB group was significantly higher than the NW group([1.11±0.62]vs[0.83±0.61]umol/L,P<0.001).Divide all the study subjects into 75 cases of male and 62 cases of female,the serum RBP4 levels of the male group was significantly higher than female group([0.99 ± 0.56]vs[0.79 ±0.69]umol/L,P=0.015).Serum RBP4 was the dependent variable and HOMA-IR age and WHR etc.were the independent variables,conducte Pearson correlation coefficient. The results showed that serum RBP4 had no significant correlation with age, WHR, TC, HDL-C, LDL-C, FPG, FINS,but it was significantly correlated with sex、BMI、TG, HOMA-IR.Conclusions:1 Divide all the study subjects into T2 DM group and NGR group,the serum RBP4 level of obesity group is significantly higher than the normal weight group,indicating that serum RBP4 plays an important role in the development of obesity.2 The serum RBP4 levels of T2 DM group is significantly higher than the NGR group,indicating that serum RBP4 plays an important role in the development of T2 DM.3 RBP4 is significantly correlated with sex、BMI、TG, indicateing that serum RBP4 plays an important role in the development of obesity through a complex pathway and reducing the level of serum RBP4 may improve the obesity.4 RBP4 is significantly correlated with HOMA-IR,indicating that serum RBP4 plays an important role in the development of T2 DM through IR.
