Objective:Discussion typing its clinical significance in gastric cancer tumor-associated macrophages; analysis of Helicobacter pylori infection in gastric cancer affects tumor associated macrophage polarization.Methods:(1)Collect the 1st affiliated hospital of bengbu medical college during May 2009 to October 2010 archive of 100 cases of gastric cancer resection group wax block, as well as 40 cases of peripheral tissues( control group),respectively the clinical pathological characteristics and survival in patients with analysis.(2)Immunohistochemical Elivision gastric carcinoma was detected 100 cases and 40 cases of peripheral tissues( control group) expression of CD16, CD163, IL-10, IL-12 protein.(3)Gram staining and immunohistochemistry Elivision were detected 100 cases of gastric carcinoma and 40 cases of peripheral tissues Helicobacter pylori(Hp)infection casesResults:1. Macrophage polarization in gastric cancer compared with the depth of tumor invasion With the increase of tumor infiltration depth of CD16+macrophages number gradually reduce and IL-12 positive expression rate gradually decreased(P<0.001),the difference was statistically significant; with the increase of tumor infiltration depth CD163+ macrophages number increase gradually and IL-10 positive expression rate increased(p<0.001), the difference was statistically significant. In gastric cancer with invasion,application of Spearman rank correlation test, Infiltrates in M2 type number of patients with cancer of the stomach, into the muscularis and serosa layer number was significantly higher in patients with the number of patients with infiltrating through the mucosa and submucosa, positively correlated between(r=0.428, P<0.001), the difference was statistically significant.2. Relationship of tumor-associated macrophages and clinical parameters According to tumor associated macrophage in gastric cancer tissue(TAMs) density distribution, the TAMs is divided into high density and low density group. Through statistical analysis of CD16+ macrophage density with the gastric cancer patients with sex,age, tumor size, lesion site, tumor differentiation degree, lymph node metastasis,tumor infiltration depth and TNM stages are no correlation(P>0.05), there was no statistically significant difference. CD163+ macrophage density height with gender,age, and patients with gastric cancer lesion had no correlation(P>0.05), there was no statistically significant difference, and in the size of the tumor, the degree of differentiation, lymph node metastases, tumor infiltrating depth and analysis found that the TNM staging gastric cancer tissues CD163+ macrophage density groups higher than that of low density groups(P<0.05), the difference was statistically significant.3. Prognostic factors in patients with gastric cancer Application Kaplan-Meier method and Cox regression analysis of patients with gastric cancer were analyzed by univariate and multivariate survival analysis, in patients with gastric cancer tumor size, tumor differentiation degree, lymph node metastases, tumor infiltrating depth, TNM staging, CD16+ macrophages and CD163+macrophage density were independent prognostic factors(P<0.05), the difference was statistically significant. And gender, age, and in patients with gastric cancer lesion location has nothing to do with the prognosis(P>0.05), there was no statistically significant difference.4. Hp test results analysis Application of Spearman rank correlation test, Hp infection in patients with tumor type M2 macrophages group obviously higher than that of M1 macrophages groups,positively correlated between(r=0.447, P<0.001), the difference was statistically significant.Conclusion:(1)In early gastric cancer is given priority to with M1 type of tumor associated macrophages, in advanced gastric cancer is given priority to with M2 type of tumor associated macrophages(2)M2 macrophages type and tumor size, differentiation, lymph node metastases,infiltrating depth, TNM staging, prompt the play an important role in the process of gastric carcinoma metastasis.(3)Hp-L may promote M1 macrophages to M2 macrophages conversions.
