A Case-Control Study Of The Influence Factors For Atrial Fibrillation

Objective To investigate the influence factors of atrial fibrillation to provide a scientific basis for government to screen the high-risk patients and make the intervention measures.Methods The patients of atrial fibrillation who had complete information and were hospitalized in department of cardiology of Luan People’s Hospital form January to May 2014 were viewed as case group. The other patients who did not have atrial fibrillation and were hospitalized in other departments of Luan People’s Hospital in the same period were randomly selected as the control group. The case-control study was conducted. Data were entered twice into a database by using the Epidata 3.1 and were analyzed using of SPSS 16.0. Univariate analysis was firstly used to screen the dependent variables, then, the unconditioned multivariate logistic regression was used to analyze the relative factors of atrial fibrillation.Results A total of 556 participants(number of case group and control group was 278, 278, respectively) were included in this analysis. Among the case group, the greatest age was 86 years old, the youngest age was 25 years old, the average age was(54.69±17.18) years old; the number of men and women was 153(55.0%) and 125(45.0%), respectively; the degree of education of primary school and below was 88(31.7%), junior high school was 50(18.0%), high school or technical secondary school was 71(25.5%), college degree or above was 69(24.8%); Among professional distribution, 88 farmer(31.7%), 139 workers(50.0%), 34 individual(12.2%), and other professions was 17(6.1%). Among the control group, the greatest age was 75 years old, the youngest age was 25 years old, the average age was(45.33±16.07) years old; the number of men and women was 112(40.3%) and 166(59.7%), respectively; the degree of education of primary school and below was 72(25.9%), junior high school was 66(23.7%), high school or technical secondary school was 60(21.6%), college degree or above was 80(28.8%); Among professional distribution, 96 farmer(34.5%), 132 workers(47.5%), 33 individual(11.9%), and other professions was 17(6.1%). The results of univariate analysis showed that the age, sex, hypertension, diabetes, coronary heart disease, heart failure, uric acid, left atrial diameter, left ventricular ejection fraction, triglycerides, high density lipoprotein and low density lipoprotein were relative with atrial fibrillation. The results of multivariate logistic regression showed that the elder age(OR=1.582, 95%CI:1.065-2.350), hypertension(OR=4.429, 95%CI:2.232-8.788), diabetes(OR=4.393, 95%CI:1.645-11.737), coronary heart disease(OR=7.517, 95%CI:2.666-21.193), heart failure(OR=10.685, 95%CI:3.481-32.802), increased uric acid(OR=1.012, 95%CI:1.005-1.020), and increased left atrial diameter(OR=1.123, 95%CI:1.092-1.155) were risk factors for atrial fibrillation; women(OR=0.501, 95%CI:0.266-0.945), an increased level of left ventricular ejection fraction(OR=0.009, 95%CI:0.001-0.054), triglycerides(OR=0.256, 95%CI:0.129-0.508), high density lipoprotein(OR=0.091, 95%CI:0.040-0.208) and low density lipoprotein(OR=0.172, 95%CI:0.076-0.390) were the protective factors for atrial fibrillation.Conclusion The occurrence of atrial fibrillation can be influenced by many factors, the patients who have the elder age, are male, have hypertension, diabetes, coronary heart disease or heart failure, or have a biochemical routine examination which shows the uric acid is increased, or have a reduced level of triglycerides, high density lipoprotein and low density lipoprotein, or have a ecg examination which shows an increased left atrial diameter and reduced left ventricular ejection fraction should pay attention to the prevention of atrial fibrillation; and the relevant departments should consider the various aspects when make the intervention measures for the atrial fibrillation.
