[Background] Adenomyosis is a common disease that discovered at 20 to 60 years old women.It is said that adenomyosis is a chronic and progressive disease that affects approximately 10% to 50% women of reproductive age.The main pathogenesis theory accept by most doctors is the endometrial glands and stroma eterotopic into the myometrium, then the lesions of cyclical bleeding with the influence of estrogen and progesterone.Most patiens with adenomyosis may have menstrual pain, menstrual blood increased, infertility, etc.At present, the treatment of uterine adenomyosis choice is sitll complex in different regions and countries, and the main treatment methods can be divided into three ways, drug therapy, surgical treatment and interventional therapy. Surgical treatment is an important way for patiens,but the recurrence of adenomyosis is still a difficult problem for clinical workers.At present our country still has no standard therapy method of uterine adenomyosis, according to statistics, about 40% - 80% of patients with adenomyosis choose surgery threatmeat, for patients with severe symptoms, older and have no fertility requirements we can choice hysterectomy as their main surgical method,but for these young and have no fertility requirements patients, we can choice partial resection of the lesions and GnRHa medication as adjuvant therapy. In this article,we selected adenomyosis patients that hospitalized in Qilu-hospital of Shandong university in 2014 as the research object, to analyze and compare the different surgical method in various adenomyosis, the details are as follows.[Objective] To analyze and compare the different surgical method in various adenomyosis, and discuss the patients’prognostic after we choice different ways.[Methods] We choiced 208 clinical cases whos’first diagnosis is adenomyosis from 1/1/2014 to 1/1/2015, in these cases,184 women choiced surgical method, then we picked up 82 cases and evaluated the effect after more than 3 months follow-up,to discuss how the age, site of lesion influence patients’pain relief, in order to draw a conclusion how to choice the surgical way in different adenomyosis cases correlated with age, site of lesion and so on.[Conclusion]①. Ccompared to open surgery, laparoscopic surgery treatment can obviously relieve patients postoperative pain (P<0.05, alpha 0.05) and average hospitalization dates (P <0.05), but it will increase average operation time compared with open surgery (P< 0.05).The intraoperative hemorrhage compared to open surgery have no statistical difference.②. Open surgery, and laparoscopic lesion resection both can do some help in patients’ pain relief, menstrual quantity decreases and prognostic factors,there is no difference in statistics (P>0.05).③. Different parts of the lesion (front wall and back wall, front wall and back wall), may have vice effect after surgical treatment.It is said that the wall of the uterus gland myopathy lesions can get better treatmeant to the anterior wall after the uterine adenomyosis lesions (P<0.05, mixed wall uterine adenomyosis lesions statistic is n’t enough, so I convince it have no statistical significance).④. The patients’age and postoperative recovery have some correlation (r= 0.84, P< 0.05), young patients would be worse in prognosis than old ones.
