BackgroundThe pathology of Nonarteritic anterior ischemic optic neuropathy is the most common cause of blindness in nerve lesions in acute optic in the middle-aged and elderly, which is closely related to hypertension, arteriosclerosis, hyperlipemia, diabetes, etc..With the changing of modern lifestyle and dietary structure, hypertension, diabetes and other diseases increase gradually, and also this disease shows a rising trend and has become one of the main diseases affecting the elderly visual function. At present, the lack of effective treatment, and insufficient understanding of the Ophthalmology physicians lead to high blinding rate and poor prognosis. Therefore, the study on the influencing factors of ischemic optic neuropathy is important.ObjectivesThe objective is to explore the influencing factors of NAION and its prevention and control measures, so as to provide a clinical basis for the diagnosis and treatment of the disease.MethodThe study is based on 324 hospitalized patients diagnosed with the pathology of nonarteritic anterior ischemic optic neuropathy in Ophthalmology hospital from January 1,2009 through December 31,2013 as a case group, and focused on 330 patients who undergo physical examination in the physical examination center as control group during the same period. From the analysis of the laboratory examination,24 hour blood pressure, systemic and general condition, the eye examination (visual acuity, fundus changes, VEP, FFA, optic discOCT, visual field, IOP), we conclude the influencing factors of this disease. We will probe into the characteristics and interactions of various factors related to the disease. The research aims to show the impact of this kind of disease prognosis through clinical intervention treatment.Main ResultsIn the case group, the average age of whom has the disease is 57.89+10.32, with male accounting for 47.84%, female 52.16%. The hypertension, high blood sugar, cholesterol, triglyceride, low ratio of hematocrit and ultrasound abnormalities of carotid artery is 49.07%,33.02%,23.53%,29.9%,8.25%,59.73%respectively. In the control group, the average age is (54.02+11.85), with male accounting for 57.58%, female accounting for 42.42%. The hypertension, high blood sugar, cholesterol, triglyceride, low ratio of hematocrit and ultrasound abnormalities of carotid artery is 23.64%,10%,16.97%,27.87%,7.57%,41.05%. In two groups, χ2 was adopted to examine high blood sugar; high blood pressure; abnormal Hematocrit. The results are statistically significant. Carotid artery anomalies was 59.73% in case group the abnormal rate of ultrasonic carotid examination was 41.05%, which is of significant difference. The 24 hours intraocular pressure and nocturnal blood pressure check show that 24 hour blood pressure in case group and the blood pressure at the heart rate monitoring point at 0:00-7:00, especially the diastolic blood pressure is lower than that in the daytime levels, at 0:00-4:00 the heart rate is lower than that in the daytime. The case group intervention patients before and after treatment were significantly improve visual acuity,, with statistically significant difference in P<0.001. Intervention of optic disc edema time of patients after treatment was obviously shortened. Eyes and the contral ateral eye disc OCT comparison,χ2=1.014E2, P<0.001, eyes and normal control group comparison of optic disc OCT, X 2=1.358E2, P<0.001, the differences had statistical significance.Conclusion and significanceNonarteritic anterior ischemic optic neuropathy is influenced by many factors, and hypertension, high blood sugar, high blood fat, low hematocrit, carotid artery anomalies, there is a close relationship between nocturnal hypotension. To further understand the vision, fundus, NAION FA, OCT, VEP etc. the optic disc features, prognosis and follow-up for patients. For non arteritic anterior ischemic optic neuropathy patients should be actively search for local and systemic causes, and to give effective intervention, can significantly shorten the course of disease, improve visual function in patients with. Make a scientific assessment of the situation of high risk population and has high incidence population prevalence, treatment and prognosis were made.
