ObjectiveThis research extracted new dialectical thinking and treatments for sepsis with integrated TCM and Western Medicine, through observing the dialectical thinking about treatment from ancient literature on the correlation between sepsis and shock, via comparing and contrasting the TCM dialectical treatment patterns between ancient medicians and modern medicians, which was expected to contribute helpful ideas to the further development of theoretical research on TCM treatment for sepsis.MethodsSearching the literature on physical symptoms and signs of septic syndrome from CNKI (China Knowledge Resource Integrated Database), the author summarized main clinical symptoms of septic shock. Based on modern medical analysis of septic shock consequences discussed in Guidelines for Management of Severe Sepsis and Septic Shock:2008 and experts’opinions, the author analyzed the highest-frequent main symptoms of modern sepsis and then summarized the mainstream ideas about the names of TCM’s counterparts to sepsis and their symptoms.With the aforementioned symptoms as keywords, I searched in the database of Zhong Hua Yi Dian (Encyclopaedia of Traditional Chinese Medicine) for a large body of related ancient literature. After meticulous reading, I picked up the most related articles on septic shock and then logged them in the TCM Inheritance Support System. Finally, the dialectical treatment rules of severe sepsis in ancient times were summed up.ResultStatistically speaking, there were altogether 14 kinds of etiological factors among 289 diagnoses. The top three ranked were damp-warm, warm- heat, and wind-warm. Among 289 diagnoses, there were a total of 53 kinds of symptoms which were listed below of a frequency over 5 times. The top three highest frequent symptoms were respectively fever, irritability, and thirst; the top three ranked tongue characteristics were greasy fur, crimson tongue, and yellow fur among 11 types in total; the top three highest frequent pulse qualities among 18 types were respectively rapid, slippery, and wiry pulses. The type of medicine used in the prescription numbered no less than 193 and the frequency of those over 20 times were displayed; the top three ranked were Forsythia, raw Rehmannia glutinosa, and fruit of Gardenia. From the pharmaceutical perspective, heat-clearing drugs were of main proportion, followed by the ones for resolving phlegm and relieving cough and asthma. Analysis from the perspective of prescription principles, the combinations of drugs were ranked from high to low frequency; "fruit of Gardenia" plus "fermented soybean", " stem of Dendrobium nobile Lindle" plus "Forsythia", "Forsythia" plus "fruit of Gardenia" ranked top three. Based on the entropy clustering, the analysis of new prescription applied unsupervised entropy hierarchical clustering algorism to the synthesis of 9 new formuli which were of core combination basis.ConclusionSeptic shock belonged to the category of warm disease in ancient times, which was mainly found in wind-warm and damp-warm diseases,in addition to "heat, poison, blood stasis, deficiency, obstruction", pathogenic wind and dampness as well were considered the cause of septic shock.In terms of treatment, the important role of heat clearing and detoxifying has always been recognized. Nevertheless, ancient physicians attached importance to eliminating dampness in expelling pathogenic factors and laid stress on nourishing body fluid in strengthening the body resistance.
