Coronary artery ectasia (CAE) is a relatively rare kind of coronary artery abnormality which can be diagnosed by coronary angiography. The etiology and pathogenesis of CAE is still not clear, and some authors regard it as a special type of coronary atherosclerosis. CAE patients without coronary stenosis may suffer coronary angina and all kinds of arrhythmia, even myocardial infarction and sudden death. CAE is veiled all around the world for its rareness. At present, there are still no systematic consensus for diagnosing and treating CAE patients.Objective:To retrospectively analyze the clinical characteristics of 669 patients with coronary artery ectasia (CAE) in Fu Wai hospital for further study of CAE.Methods:From January 1st 2009 to May 5th 2014,106305 consecutive patients underwent coronary angiography at Fu Wai hospital. The clinical characteristics of 669 CAE patients with intact medical records were analyzed.Results:The prevalence of CAE was 0.656%, and 69 isolated CAE patients were observed, taking up 10.31% of the total CAE patients. Of the CAE patients,84.75% were male(totally 567), the ratio of male and female was about 5.56, the patients were between 16-84 years old, with the mean age(57.16±10.972)years old.31.99% CAE patients (214) were smokers,34.68% (232) with hypertension,31.54% (211) with hyperlipidemia, and 11.36% (76) with diabetes.22.12% CAE patients had previous myocardial infarction, and 23.77% had undergone coronary stent implantation previously. There were totally 878 dilated coronary arteries, and the main branch of right coronary artery was mostly involved (totally 367,41.80%), followed by the main branch of left anterior descending artery (totally 223,25.40%), the main branch of left circumflex artery (totally 190, 21.64%), and the left main coronary artery (totally 42,4.78%). The laboratory tests showed that,42.90% CAE patients had higher triglyceride level,71.60% had higher low-density lipoprotein cholesterol level,31.99% had higher lipoprotein(a) level,36.62% CAE patients had higher high-sensitivity C-reactive protein level, and 26.31% had lower cholesterol level.Conclusions:CAE has relatively rare prevalence rate with the main branch of right coronary artery mostly involved, and men are mostly involved. The risk factors of CAE are similar as coronary atherosclerotic disease except diabetes. Most CAE patients have abnormal blood lipid level, and CAE may relate to inflammation. Also, CAE and coronary atherosclerosis may be two different kinds of abnomalities.
