Objective Realize the changes of pelvic ultrasonography and bone age index(BAI) in girls with precocious puberty, estimate their value in the early diagnosis, and provide some basis for the girls’ first diagnosis, especially in the early diagnosis of outpatient girls.Method Eighty-four diagnosed precocious puberty girls were enrolled, collected all the information. All cases were performed pelvic ultrasound, measure and calculated the uterine length and volume, ovarian length and volume, the follicle number and follicle diameter; took X-ray film, evaluated and calculated the bone age index. Hormonal evaluation included basal levels of luteinizing hormone(LH), follicle-stimulating hormone(FSH) and estradiol(E2) as well as LH and FSH levels after Gn RHa-stimulated text using immune chemiluminescence measurement. Using Epi Data3.1 to input datas accurately, SPSS19.0 software for statistical analysis.The t-test was used to compare the mean value between two independent groups. As to ovarian morphology, significance between percentages in two groups was assessed using the Chi-square test. Pearson correlation was used to analysis and compare the relationship of the uterine length and volume, the ovarian length and volume, the BAI, sex hormone levels before and after Gn RHa-stimulated text. A P-value less than 0.05 was considered statistically significant. Finally, receiver-operating characteristics(ROC) curves was used to compare areas under the ROC curve(Az), and to select optimal cut-off points for discriminating between different forms of sexual precocity, then evaluate value of uterus, ovarian volume and BAI in girls with precocious puberty.Results Uterine length, uterine volume, ovarian length, ovarian volume, BAI and results of Gn RHa stimulate test(LH peak, LH peak/FSH peak) were high in girls with idiopathic central precocious(ICPP) than girls with peripheral precocious puberty(PPP); the uterine length, uterine volume, ovarian length, ovarian volume and BAI was significantly associated with LH peak, LH peak/FSH peak in ICPP girls, and no correlation with PPP girls; areas under RCO curve of uterus length, uterus volume, ovary length, ovarian volume, follicle diameter and BAI respectively were 0.83, 0.96, 0.86, 088, 0.94, 0.84; among those dates, uterine volume and maximum follicle diameter were supreme for early diagnosis of girls with idiopathic central precocious puberty.Conclusions Uterine and ovarian development is relatively mature in girls with idiopathic central precocious puberty group, and the development were positively correlated with gonad axis functions; Among them, the volume of uterus and largest follicle diameter is of relatively great value in diagnosis of idiopathic central precocious puberty. The BAI were higher in girls with idiopathic central precocious puberty than girls with peripheral precocious puberty, and BAI had certain reference significance to the diagnosis of idiopathic precocious puberty. The uterus, ovarian ultrasound and BAI all valuable for early diagnosis of girls with idiopathic central precocious puberty. For girls who had lager uterine volume or increased maximum follicle diameter should complete the relevant inspection as soon as possible, in case misdiagnosis.
