Objective: This experiment mainly through the observation and analysis of the characteristics of subcortical ischemic vascular cognitive impairment(SIVCI) in patients with memory component damage, and changes in the characteristics of event related potentials in patients with SIVCI. To explore the neuropsychological and event related potential application value in the early diagnosis of SIVCI. To improve the understanding of damage in patients with SIVCI of memory and the early diagnosis of SIVCI level. Methods: 30 cases of SIVCIND patients, 30 cases of SIVD patients, 30 cases of gender, age, education level matched normal control group. They were finish some testing: 1. The overall cognitive function respectively by MoCA and MMSE were detected in three groups. 2. Computer aided testing: Test the item memory(Pr value) and source memory(I value); 3. Event-related potentials: Determination of mismatch negativity(MMN) and P300; 4. The correlation analysis. Results: 1. SIVCIND group and NC group for comparison, SIVCIND group delayed memory, attention and executive declined, there were significant gaps(P <0.05), and language ability, abstract, directional force, naming ability no significant decrease(P >0.05);SIVD group and NC group for comparison, SIVD group delayed recall, visual spatial and executive, attention, orientation, abstract decreased,(P <0.05), language ability,naming ability no significant change(P>0.05);2.Neural psychology test: Item memory test and source memory test: ? SIVCIND group and NC group for comparison, the Pr value decreased, there were statistical significance difference(P <0.05); SIVD group and NC group, the Pr value decreased obviously, the gap had statistical significance(P <0.05); SIVD group and SIVCIND group for comparison, the Pr value decreased, there were statistical significance difference(P <0.05). ?SIVCIND group and NC group for comparison, the I value decreased, there were statistical significance difference(P <0.05); SIVD group and NC group for comparison, the I value decreased obviously, the gap had statistical significance(P <0.05); SIVD group and SIVCIND group for comparison, the I value decreased, there were statistical significance difference(P <0.05). ?The results showed that source memory and item memory damaged in patients with SIVCIND group and SIVD group. The damage of SIVD group was more than SIVCIND group obviously. 3. The event related potential: ?SIVCIND group compared with NC group, the P3 latency was prolonged(P <0.05) and the amplitude decreased(P <0.05); SIVD group compared with NC group, the P3 latency was prolonged(P <0.01) and the amplitude of P3 decreased significantly(P <0.01);SIVD group compared with SIVCIND group, the P3 latency was prolonged(P <0.05) and the amplitude decreased(P <0.05).?SIVCIND group compared with NC group, the MMN latency was prolonged(P <0.05) and the amplitude decreased(P <0.05); SIVD group compared with NC group, the MMN latency was significantly prolonged(P <0.01) and P3 wave amplitude decreased significantly(P <0.01);SIVD group compared with SIVCIND group, the MMN latency was prolonged(P <0.05) and the amplitude decreased(P <0.05).4.Correlation analysis of P300, MMN and neural psychology test: ? The latency of P300 and Pr value was negatively correlated; the amplitude of P300 were positively correlated with Pr value; the latency of MMN and Pr value was negatively correlated; the amplitude of MMN were positively correlated with Pr value.?The latency of P300 and I value was negatively correlated; the amplitude of P300 was positively correlated with I value; the latency of MMN and I value was negatively correlated; the amplitude of MMN was positively correlated with I value; the results show that P300, MMN latency and amplitude and I value have good correlation. Conclusion: 1. The discovery of general cognitive function evaluation, SIVCIND delayed recall, executive, attention and other cognitive function decreased. But the SIVD delayed recall, visual spatial and executive, attention, orientation force, abstract decreased. 2. The item memory, source memory of SIVCIND group and SIVD group were dysfunction, the damage of SIVD group was more than SIVCIND group obviously. 3. Both in SIVCIND group and SIVD group, P300 and MMN showed prolonged latency, amplitude decreased, and SIVD group was more obviously. 4. P300, MMN and item memory, source memory have good correlation. P300, MMN could be used as a neurophysiological evaluation of SIVCI patients with the item memory and source memory impairment.
