Objective: To introduce a new surgical treatment of chronic subdural hematoma which is most prevalent in the elderly patient and is the most common conditions seen in daily neurosurgical practice,micro drill catheter wash and evaluate it’s characteristic and effect.Methods:128 patients with Chronic subdural hematoma treated in our Hospital from February,2009 to December, 2014 were retrospectively study. 128 patients were divided into the experimental group and the control group.86 patients of the experimental group were treated with micro一drill catheter wash.42 patients of the control group were treated with YL-1 hard passage drainage.There were no statistical difference in age、sex and basic diseases of the two groups.76 patients of the experimental group were male,10 of them were female,with a mean of age 71.7 years,range from 35 to 90.The CSDH was one side in 73 cases, and bilateral in the remaining 13 cases,a total of 99 side. 69 cases had definite head trauma. All cases were treated with micro一drill catheter wash. After operation, drainage tube was retention and washed blood intermittent. According to CT results extubated timely. Generally the indwelling time was 2~3 days,and washed 2~3 times. After operation, according to the modified Markwalder score to evaluated the therapeutic effect of CSDH. 36 patients of the control group were male,6 of them were female,with a mean of age 70.6 years,range from 36 to 88;The CSDH was one side in 35 cases, and bilateral in the remaining 7 cases,a total of 49 side. 28 cases had definite head trauma. All cases were treated with YL-1 hard passage drainage. After operation, drainage tube was retention and washed blood intermittent. According to CT results extubated timely. Generally the indwelling time was 3~4 days,and washed 2~3 times. After operation, according to the modified Markwalder score to evaluated the therapeutic effect of CSDH.Results:78 cases(90.7) were cured by washed once and 8 cases(9.3%) improved.The total efficiendcy was 100%. 4 cases recurrented(4.6%), 0 cases of intracranial infection occurred,1 cases occurred postoperative epidural hematoma(1.1%), tension pneumocephalus was 0 cases. 28 cases(66.7%) were cured by washed once and 9 cases(21.4%) improved.The total efficiendcy was 88%. 8 cases(19%) recurrented, 2 cases(4.7%) of intracranial infection occurred,3 cases(7.1%) occurred postoperative epidural hematoma, 2 cases(4.7%) occurred intracranial hematoma, tension pneumocephalus was 17 cases(40.4%).Conclusion:Micro-drill catheter wash is minimally invasive operation, avoiding operative complication effectively, providing minimally invasive, simple operation, a satisfactory effect and a low recurrence rate compared with the traditional minimally invasive operation in treatment of chronic subdural hematoma.
