Objective By making comprehensive analysis and comparative study on the ruralresident’s health services utilization, coefficient of elasticity both before and after regulatingthe New Rural Cooperative System, the author intends to analysis of rural resident’s healthservices utilization status and trends. Evaluate the effect of program, put forward targetedpolicy recommendations, the results would provide some scientific evidences and suggestionsfor perfect and sustainable development of health reform system.Methods Using the stratified random clusters sampling and homemadequestionnaires were used to select participants and collect research data. And in the years of2011and2012mad the follow-up visit of the participants.Results In Haiyuan county, the two-week prevalence rates were17.15%,16.41%,11.76%; the chronic illness prevalence rates were13.28%,9.71%,12.28%; Two-weekconsultation rates were10.69%,10.15%,7.12%; the hospitalization rates were8.48%,8.76%,9.39%. In Yanchi county, the two-week prevalence rates were14.14%,7.32%,15.50%; thechronic illness prevalence rates were14.39%,9.56%,16.97%; Two-week consultation rateswere9.15%,6.64%,9.75%; the hospitalization rates were10.30%,9.54%,11.07%.Two-weeknon-consultation rates and the non-hospitalization rates were show a clear descending trendin the last three years, the main reason is from the financial difficulties to the self-inductanceof mild disease for persons who were not going to a doctor, the proportion of not hospitalizedbecause of financial difficulties. According to the multilevel model fitting result shows that main factors related tothe prevalence of the rural residents in last two weeks were gender, age, education level,marital status and whether or not suffering chronic disease,and main factors related to thechronic illness prevalence of the rural residents were gender, age, education level, maritalstatus and annual average income of the family; the factors influencing participant visitingdoctor in past two weeks were whether or not suffering from chronic disease and days ofbedridden due to the illness; main factors influencing participant received hospitalizedtreatment were ethnicity and whether or not suffering chronic disease.In Haiyuan county, the marginal propensity to consume of healthcare wererespectively0.009,0.012and0.010; the income elasticity of demand for medical service were0.06,0.08,0.07; the basic needs of medical services expenditures were464.37yuan,630.02yuan,665.69yuan, which shows a clear ascending trend in the last three years. In Yanchicounty, the marginal propensity to consume of healthcare were respectively0.006,0.004and0.010; the income elasticity of demand for medical service were0.06,0.06,0.10; the basicneeds of medical services expenditures were476.16yuan,525.72yuan,766.67yuan, whichalso shows a clear ascending trend in the last three years. In two counties, there are growingtendency of rising proportion of medical cost in personal consumption, that proportion ishigher in the poor group than that in the rich one.Conclusion The enforcing new scheme has not a great influence on outpatient clinicservice, but has had an impact on the medical service utilization in hospital to some extent,the situation of persons not going to a doctor and not hospitalized has improved whichbecause of financial difficulties. As many factors affecting the health service needs andutilization of rural residents, health policy makers should pay attention to these factors tomake reasonable health resources allocation, for the new program make some factors change,this result need for further research. After enforcing the new type of rural cooperative medicalsystem, the income elasticity of demand for medical services still showed low elastic state, although the income does affect the demand for medical service, but the increase doesn’tmean the rural residents pay the equal proportion for medical service, and they start to payattention to the consume of the medical service.
