Total Hip Arthroplasty (THA) is an effective treatment for end-stage hiposteoarthritis. However, the surgery is associated with considerablecomplication, included blood loss. Now,this study is to evaluate the effectivenessand safety of intravenous tranexamic acid in primary total hip arthroplasty forthe patients with osteoarthritis. Objective:To evaluate the effectiveness and safety of intravenous tranexamicacid in primary total hip arthroplasty for the patients with osteoarthritis. Methods: This study was based on Cochrane methodology for conductingmeta-analyses. Only randomized controlled trials were eligible for this study. The participants were the adults who suffered from osteoarthritis, excluderheumatoid arthritis, osteonecrosis, fracture, had undergone THA. The literaturesearch was conducted using the Pubmed,Cochrane Library,MEDLINE and ChinaNational Knowledge Infrastructrue(CNKI) databases. RevMan5.2was used toanalyze selected studies. Results: Seven randomized controlled trials involving458patients wereincluded. The use of TXA reduced postoperative drainage by a mean of194mL(95%confdence interval(CI)[177.3,211.16], total blood loss by a mean of284.22ml(95%CI[143.44,424.99]),and transfusion rate (RR0.6095%CI [0.42,0.87]). There were no signifcant differences in the incidence of venousthromboembolism(VTE)、pulmonary thromboembolism(PTE)、operative time、intraoperative blood loss and transfusion unit per patient among thestudy groups. Conclusions: Intravenous TXA could significantly reduce postoperative bloodloss and blood transfusion rate following THA for the patients with osteoarthritis, but not intraoperative blood loss、transfusion unit per patient and operative time. Its application did not increase risk of VTE or PTE。... |
