Objective To explore the changes of serum Golgi-protein73(GP73) single andcombined with alpha fetoprotein (AFP) detection in HBsAg-positive primary hepatocellularcarcinoma and its clinical significance.Methods Select32cases of chronic HBV carriers(Carriers Group),35cases of chronichepatitis B(Hepatitis Group),40cases of liver cirrhosis(Liver Cirrhosis Group),45cases ofprimary hepatocellular carcinoma(Hepatocellular Carcinoma Group),all the patients,hepatitisB surface antigen (HBsAg) were positive.The other30cases of he-althy people were selectedas control group,The levels of serum GP73were measured by ELISA,using electrochemicalluminescence method to detect the level of serum AFP.At the same time to detect the ALT,ALP, CHE, GGT, HBV-DNA (except control groupt) index,statistical analysis of the testresults.Results (1) The serum GP73in healthy controls,HBV carriers, chronic active hepatitis B,liver cirrhosis,primary hepatocellular carcinoma patients can be measured,thelevels of serum GP73in different conditions were diff-erent, the level of GP73in healthy control group was the lowest (49.64±12.30)ng/ml.Chronic HBV carriers group(73.76±15.35)ng/ml,chronic hepatitis B group(112.26±13.39)ng/ml,hepatitis B cirrhosis group(142.63±13.30)ng/ml,pri-mary hepatic carcinoma group(161.36±26.02) ng/ml,The level of serum GP73increased gradually with the progress of the disease,each the different two groups was statistically significant (P<0.05);(2) Tthrough detectioning the serum GP73and AFP,drawing ROC curve,the best diagnostic boundaries of serum GP73was138.45ng/ml, the sensitivity of prediction was82.2%, the specificity was78.9%;th e best diagnostic boundaries of serum AFP was316.61ng/ml,the sensitivity was62.2%,specificity was69.3%,Area under the ROC curve (AUC) in diagnosis of PHC, GP73was0.907larger than the AFP,0.798,95%confidence interval were0.762~0.951and0.664~0.793, the AUC has significantly difference (P<0.05).Serum GP73≥138.45ng/ml asposi-tive,serum AFP≥316.61ng/ml as positive, the combined detection sensitivity up to93.3%,was higher than that of serum AFP or GP73single detection (P<0.05);co-mbined detection specificity (54.7%) was lower than that of ser-um AFP or GP73singledetection(P<0.05);(3) Through the Pearson correla-tion analysis, serum GP73has a certain correlation with ALT,ALP,CHE,GGT, the r values were0.426,0.680,0.524,0.568(P<0.05); a low correlation with HBV-DNA(r=0.214,P<0.05);but the serum GP73and AFP,by Pearson correlation analysis,were no correlation(P>0.05,r=0.454).Conclusion The of serum GP73can be used as a better diagnostic serum marker ofPHC,the two combined detection has a higher sensitivity,which can improve the diagnosis ofprimary hepatocellular carcinoma rates;But the expression of serum GP73and AFP were nocorrelation,GP73expression and regulation may be different from the AFP,To the lowconcentration of AFP or negative PHC patients,GP73has certain clinical value can be used asa supplementary diagnostic index.
