| Objective: To recognize and prevent the clinical influential factors in ARDS duo toinfection early, reduce mortality and improve the cure rate, we collected the clinical data ofpatients with ARDS in ICU of Ningxia Medical University General Hospital and analyzed theinfluential factors that led to ARDS affected disease progression and prognosis.Methods: The risk factors for prognosis in52patient with ARDS due to infectionadimitted to our hospital from March2010to December2012were analyzedretrospectively.The patients were divided into two groups:Survivors group (n=27) and Deathgroup (n=25) on the basis of prognosis. Etiology,age, gender, chronic disease, acute organdysfunction, blood-gas analysis, APACHE II score of patients, ALB, BUN, the variety ofantibiotics,the distribution of bacterial species in the two groups were analyzed. Statisticalanalyses was done using SPSS17.0statistical software. Data are expressed as mean±SD. Alldata were analyzed by univariate test, one-way analysis of variance and multiple logisticregression method.Results:①Single factor analysis showed that chronic disease(P=0.036), pulmonaryinfection(P=0.011), cardiovascular system failure(P=0.015), gastrointestinal tract failure(P=0.038), blood urea nitrogen(P=0.032), albumin(P=0.028), Candida albicans infection(P=0.023) were the risk factors for the prognosis ofARDS due to pulmonary infection.②Multivariate logistic regression analysis showed that pulmonary infection(P=0.016,OR:2.203,OR95%CI:1.156-4.201), cardiovascular system failure(P=0.010,OR:1.149,OR95%CI:1.034-1.278), Candida albicans infection(P=0.046,OR:0.994,OR95%CI:0.988-1.000) were the indepentdent risk factors for the prognosis of ARDS due to pulmonaryinfection.110strains of pathogenic bacteria have been detected in the52patients of ARDSdue to infection. The top five of the detected110strains bacteria is Acinetobacter baumannii(19.1%), Candida albicans (11.9%),Pseudomonas aeruginosa(10.9%), Escherichiacoli(10.0%),Klebsiella pneumoniae(8.2%).Conclusion:(1) Chronic disease, pulmonary infection, cardiovascular system failure,blood urea nitrogen, albumin, Candida albicans infection can affect the prognosis of ARDSdue to pulmonary infection.(2) Pulmonary infection, acute cardiovascular system failure,Candida al-bicans infection were the indepentdent risk factors for the prognosis of ARDS dueto pulmonary infection. |
