Objective To analyse the clinical effect of monosegment pedicle instrumentation withthose of shortsegment pedicle instrumentation and provide an appropriate selection of surgicaltreatment of thoracolumbar fracture.Method30patients with thoracolumbar fractures divide posterior mono-segment t withshort segmental pedicle internal fixation,each group15cases,Measured preoperative andpostoperative surgery X-ray, Comparing significant difference of fracture vertebral bodyheight,correct rate of edge,correct rate of Cobb’S angle.Results Short-segment and mono-segment cases were followed up for6months, itfound that a total of five cases of vertebral height lost (which single segment were3cases,2cases of short segments),3cases after Cobb’S angle increased slightly convex (which singlesegment group2patients, short segment group1patients),it not found implant failure,broken nails.After6months later it found that the two groups of fixation of correct rate ofvertebral compression and correct rate of Cobb’S angle than before surgery were statisticallysignificant, and which two group vertebral height and kyphosis Cobb’S angle were significantimprovement compared with the preoperative, they were statistically significant comparedpreoperative and postoperative. Short-segment fixation group regardless of time or loss ofblood volume is larger than mono-segment fixation group; the two groups have differencessignificant. There was no difference significant of Frankel classification in the preoperative,were statistically significant for Frankel classification on postoperative and follow-up witheach group compared preoperative. VAS Score were no differences significant for two groupspreoperative, postoperative and follow-up wereb the same score (VAS score) no difference significant, was not statistically significant. However, compared with the pain score wasdifferent significantly within the two groups for surgery.Conclusion Mono-segment and short-segment fixation treatment of thoracolumbarfractures,which have a certain effects in fracturesof thoracolumbar, kyphosis correction,restore vertebral height and,improve back pain. Although mono-segment groups has betterthan short segment group in blood loss and incision length, operation costs are less, but theshort segment group for recovery and restore vertebral body height or kyphosis cobbs cornerefficacy is superior to monosegment group in postoperative and long-term effects. There haveadvantages and disadvantages of both methods can treat thoracolumbar fractures, just have agood indication, can get a good result.
