Object To study the value of3.0T MR LAVA technology combined DWI on detectionand qualitation of small hepatocellular carcinoma (SHCC) with cirrhosis backgound.MethodCollected107cases SHCC patients information with cirrhosis, all leisions were confirmed bypathology or the DSA. Lesions were found in119, single lesion95, double lesions12.Allpatients underwent T1WI(positive and negative phase), T2WI, multiphase dynamic enhancedscan(LAVA), DWI scan(b值800s/mm2). Comparison and analysis of T1WI, T2WI, DWI,LAVA enhanced each phase and LAVA enhance jointed DWI and LAVA enhanced indetection rate of SHCC; T1WI, T2WI, LAVA enhanced (group A), T1WI, T2WI, DWI(group B), their combined (group C) in qualitation value of SHCC. Observed signal,statistical comparison of SHCC pseudocapsules was displayed on each sequence; depicted thelesion and surrounding liver tissue time signal intensity curves, measurement of leision and itssurrounding liver tissue ADC value. Result1LAVA enhanced detection rate of SHCCwas higher than DWI, T2WI, T1WI, but LAVA enhanced compared with DWI have nosignificant difference.2LAVA arterial phase detection of SHCC compared with portal orequilibrium phase was higher,have significant difference.3LAVA enhanced displayed SHCCpseudocapsules (quantity and complete) were more than T1WI, T2WI;T1WI was more thanT2WI.4LAVA enhanced jointed DWI detection rate of SHCC was higher than LAVAenhanced, but there was no statistical difference between the two.5Group C accuratecharacterization of SHCC was higher than group A, group B,with a significant difference.6SHCC ADC value was lower than the surrounding liver tissue,with a significant difference between the two.Conclusion1LAVA enhanced compared with DWI, T2WI, T1WI canbetter detection of SHCC.2LAVA enhanced arterial phase detection of SHCC is veryimportan on cirrhosis background.3LAVA enhancement (portal phase+equilibrium phase)show SHCC pseudocapsules (quantity, integrity) better than T1WI, T2WI;T1WI is better thanT2WI.4LAVA enhanced joint DWI can reduce the small hepatocellular carcinoma missed,DWI is a useful supplement to LAVA enhanced.5LAVA enhanced joint DWI can help toimprove the accuracy qualitation of SHCC on cirrhosis background.6The study of smallhepatocellular carcinoma ADC value is lower than the surrounding liver tissue, indicatingrestricted diffusion of SHCC.
