Research ObjectivesThe study aims to use the discrete choice experiment(sDCE)to analyze current primaryhealth care staff working preferences so as to work out the incentive factors that affect thebasic health workers. The final purpose is to provide the policy basis for establishing areasonable basic health incentive policies and perfecting human resources health systems.Research Content1Utilize various conditions, factors and the level of target audience to define the DCEmethod.2Confirm the factors that influence the basic health staff motivation through literaturereview, interviews with experts.3The analysis and exploration of the DCE method toward basic health workerspreference.Research MethodsIn this study, mainly using DCE (discrete choice experiment) method to test and confirmthe grassroots health workers’ will and motivation factors towards work Methods used includeliterature review, Delphi method, questionnaire, orthogonal, clog itregression analysisresearch, which are required to investigate the situation of the basic health worker andmotivation incentives in Ningxia so as to define the factors that affect the grassroots healthworkers’ work options and motivation..Research Results1. In the first turn of expert consultation.The coordination coefficient of the specialists of importance of the five first indexes are0.400,0.261,0.357,0.302,0.389and total indexd is0.329. The value of P is less than0.05by Chi-square test. In the second turn of expertconsultation, the coordination coefficient of the specialists of importance of the five firstindexes are0.448,0.447,0.386,0.402,0.411and total index is0.410, The value of P is lessthan0.05by Chi-square test.The degree of the coordination coefficient in the second turn isbetter than in the fist turn.The result is appropriate.2. Based on the literature review and expert advice, the grassroots health workers’incentives in Ningxia is defined. It includes five first-level indicators: salaries, social security,working conditions, human environment, self-realization and eight secondary indicators:monthly income and benefits, workplace and children’s attending, incentives, preparation,training opportunities, work equipment, job promotion time.3. DCE survey results show that Indicators that impact the grassroots health workersworking preference can be ranked from high level to low as monthly income and allowances,preparation, workplace, work equipment, job promotion time, solution to their childrenattending, incentives, training opportunities. The indicators are meaningful and in positivecorrelation, and it plays a significant role in motivating the grassroots health workers. Policymakers can formulate incentive policies in accordance with these indicators.4. Survey of different institutions,different professions, different ages, different genders,different registered residence, different academic target make for different incentives cases.Therefore, the policy makers should formulate appropriate incentives based on the actualsituation.5. Knowing from qualitative interviews, grassroot health workers are confined becauseof the state policy.The basic health institutions are not allowed to establish clinicalmedicine,.Therefore, the grassroot health workers can not put what they have learned intopractice.and bring out their strong point into full play. Besides, the ethic views of the health works are distorted.They focus more on material.They are unclear about the states aid policyand under the inadequate protection and incentives.What’s worse, grassroot health workersget low incomes with poor working conditions, less training opportunities, work load, whichlead to a severe shortage of talent, a serious drain on the impact grass-roots health institutions.Government lack of effective supervision and management mechanism. The effect ofincentives is not obvious, and training and learning opportunities for grassroot health workersis not enough.Suggestions1. Establish a fair, impartial and reasonable compensation system.2. Establish a scientific performance appraisal system and standardized assessmentprocedures.3. Perfect the medical education system to fit Ningxia grassroots health institutions staffdevelopment.4. Improving grassroot health institutions working environment.5. Increase Ningxia grassroot health establishment, and give the priority of the policysupport on the job promotion6. Aim at different requitements to carry out the individual motivation incentives.
