Objectives To observe the influence of carbon dioxide pneumoperitoneum withdifferent pressure and inflation time on peritoneal morphology and ovarian morphologyof the rat.Methods330in diestrus SD rats of10weeks oid were randomly divided into the blankcontrol group, the pneumoperitoneum group, and the non pneumoperitoneum group.Every group was given different pressure(10mmHg,15mmHg,20mmHg), and differentinflation time(60min,120min), each group was10rats. The parietal peritoneum and bothovaries were resected from every group rats in0hours,6hours,12hours,24hours afteroperation. The morphological changes were observed under light microscope andtransmission electron microscope.Results1the changes of peritoneal morphology under light microscope: afterpneumoperitoneum in the group of the pressure of15mmHg, the inflation time of60min,the gap between the mesothelial cells increased at0h. At6h, connection between themesothelial cells was destroyed. Dilatation of blood vessels, infiltration of inflammatorycells was observed too. The changes were more obvious at12h. At24h, the hyperplasiaof mesothelial cells and thickening of peritoneum was observed. And the difference wasstatistically significant. The changes of the pneumoperitoneum group under the samepreesure for inflation time of120min were more obvious than inflation time of120min(P<0.05). With the same time of inflation pressure, the changes of were more obvious inthe group of high pressure of pneumoperitoneum (P<0.017). Compared with the blankcontrol group and non pneumoperitoneum group the changes of the pneumoperitoneumgroup were obvious significantly(P<0.01);2the changes of peritoneal morphology undertransmission electron microscope: after pneumoperitoneum in the group of the pressureof15mmHg, the inflation time of60min, at0h the gap between cells increased, microvillidistorted and mitochondria was mild edema. At6h,basement of the mesothelial cells wasdestroyed, the number was decreased, and the microvilli was destroyed too. At the sametime, the edema of mitochondria is more obvious. Vacuolated mitochondria anddilatation of endoplasmic reticulum was observed following the time of12h. At24h, thenew mesothelial cells were found. And the difference was statistically significant. Thechanges of the pneumoperi-toneum group under the same preesure for inflation time of120min were more obvious than inflation time of120min (P<0.05). With the same timeof inflation pressure, the changes of were more obvious in the group of high pressure ofpneumoperitoneum (P<0.017). Compared with the blank control group and nonpneumoperitoneum group the changes of the pneumoperitoneum group were obvioussignificantly(P<0.01);3the changes of ovarian morphology under light microscope:after pneumoperiton-eum in the group of the pressure of15mmHg, the inflation time of60min, at0h, interstitial cell was mild edema and A small amount of inflammatory cellwas infiltrated. At6h, interstitial cells and granulosa cells were mild edema, bloodvessels were dilatated, inflammatory cell was infiltrated. The changes were more obviousat12h. The changes were the most obvious at24h. And the difference was statisticallysignificant. The changes of the pneumoperitoneum group under the same preesure for inflation time of120min were more obvious than inflation time of120min (P<0.05).With the same time of inflation pressure, the changes of were more obvious in the groupof high pressure of pneumoperitoneum(P<0.017). Mild edema of mitochondria, cristae shortened,dilatation of some endoplasmic reticulum of theca cells and granulosa cells wereobserved at6h. At12h, the edema of mitochondria was aggravated, most cristaedisappeared and the dilatation of some endoplasmic reticulum was obvious. At24hours,most of the mitochondria were vacuolated, the changes of endoplasmic reticulum weremore obvious, the number of lipid droplets was increased. And the difference wasstatistically significant. The changes of the pneumoperitoneum group under the samepreesure for inflation time of120min were more obvious than inflation time of120min(P<0.05). With the same time of inflation pressure, the changes of were more obvious inthe group of high pressure of pneumoperitoneum (P<0.05). Compared with the blankcontrol group and non pneumoperitoneum group the changes of the pneumoperitoneumgroup were obvious significantly(P<0.01).Conclusions1After carbon dioxide pneumoperitoneum, with the time going, the gapbetween peritoneal mesothelial cells increased under light microscope, then connectionbetween the mesothelial cells was destroyed. Dilatation of blood vessels, infiltration ofinflammatory cells was observed too. At24h, the hyperplasia of mesothelial cells andthickening of peritoneum was observed. Under transmission electron microscope, thegap between cells increased, microvilli distorted, and was destroyed, basement of themesothelial cells was destroyed. The edema of mitochondria and dilatation ofendoplasmic reticulum was observed. At24h, the new mesothelial cells were found. Thehigher pressure and longer operation time with laparoscopic operation, the more obviousthe changes of peritoneal morphology are.;2After carbon dioxide pneumoperitoneum,under light microscope, interstitial cells, granulosa cells and theca cells were edema,blood vessels were dilatated, inflammatory cell was infiltrated. Under transmissionelectron microscope, edema of mitochondria and dilatation of endoplasmic reticulumwas observed. The changes were more obvious with the time going. The higher pressureand longer operation time with laparoscopic operation, the more obvious the changes ofovarian morphology are.
