| Part I64Slices Helical CT Perfusion Imaging for Normal PancreasObjective:To explore the characteristics of normal pancreatic CT perfusion imaging and to find out the value of perfusion parameters,which is the base for the following study. Methods:45cases without pancreatic diseases were examined for pancreatic CT perfusion from Feburery2010to March2011,with continuous volume scan pattern,120Kv,150mA,512x512pixels, on a Siemens Sensation64CT scanner. The contract agent Omnipaque30050mL was injected into the median cubital vein at the rate of5mL/sec, with a delay time of6sec, data a collection time of40s, a slice width of7.2mm(4slices/3cm). The perfusion CT scan date was processed on Syngo Multi-Modality Workplace. The mean BF,BV,TTP and PS were measured and statistically analyzed. Results:The mean values of BF,BV,TTP and PS of normal pancreas were128.06±23.48ml·min-1·100g-1,203.38±33.24ml·kg-1,143.45±14.66sec,64.77±15.35ml·min-1·100g-1respectively, which symmetrical distribution in whole pancreas. Conclusion:The normal pancreas is an symmetrical perfusion organ symmetrical with parameters of BF, B V, TTP and PS. Part II CT Perfusion Imaging for Pancreatic NeoplasmsObjective:To explore the characteristics of CT perfusion imaging in pancreatic neoplasms, to evaluate the value of CT perfusion in the diagnosis of pancreatic neoplasms. Methods:51cases with pancreatic neoplasms were examined for pancreatic CT perfusion from Feburery2010to March2011, with continuous volume scan pattern,120Kv,150mA,512x512pixels, on a Siemens Sensation64CT scanner. Results:1.The mean value of BF, BV, TTP and PS of pancreatic carcinoma were35.97±15.54ml-min-1·100g-1,66.45±26.75ml·kg-1,158.13±32.04sec,37.67±15.84ml·min-1·100g-1respectively. Comparing with the normal pancreas, pancreatic carcinoma had lower BF,BV,PS and higher TTP.2. The mean values of BF,BV,TTP and PS of solid-pseudopapillary tumor of pancreas were49.76±21.41ml·min-1·100g-1,65.33±18.90ml·kg-1,138.86±38.39sec,36.23±15.05ml·min-1·100g-1respectively. Comparing with the normal pancreas, solid-pseudo-papillary tumor of pancreas had lower BF, BV and PS.3. The mean value of BF, BV, TTP and PS of islet cell tumor were302.62±66.65ml·min-1·100g-1,380.83±95.80ml·kg-1,149.27±22.53sec,77.85+11.62ml·min-1·100g-1respectively. Comparing with the normal pancreas, islet cell tumor had higher BF and BV.4. The mean value of BF, BV, TTP and PS of intraductal papillary mucinous neoplasm of pancreas were27.9ml·min-1·100g-1,20.3ml·kg-1,96.8sec,7.8ml·min-1·100g-1respectively. Conclusion:1. Pancreatic carcinoma is a malignant tumor with low BF, BV, PS.2.solid-pseudopapillary tumor of pancreas is a poor blood supply tumor with low BV and PS.3. Islet cell tumor is a rich blood supply tumor with high BF and BV. Part Ⅲ Correlations of CT Perfusion Imaging and the Angiogenesis of the Pancreatic CarcinomaObjective:To analyze the correlation of CT perfusion,VEGF and MVD. Combined with preoperational pancreatic CT perfusion,the values of functional CT imaging applied to evaluate the angiogenesis of pancreatic carcinoma were investigated.Methods:23pancreatic carcinoma specimens were studied.MVD and VEGF were measured with mean of five microvessel views.Results:1.The positive rate of VEGF were82.61%and8.70%in pancreatic carcinoma and normal pancreatic tissue respectively.The mean MVD were22.30±9.64in pancreatic carcinoma and11.74±6.21in normal pancreatic tissue.The mean MVD of pancreatic carcinoma was higher than normal pancreatic tissue.2.There was correlation between VEGF and MVD in pancreatic carcinoma with a r value of0.66and a p value of0.05.3.The mean MVD of pancreatic carcinoma existed a negative correlation with BF, BV.Also a negative correlation exhibited between the values of BF and VEGF.Conclusion:1.Expression of VEGF and MVD in pancreatic carcinoma and normal pancreatic tissue were detected.2.Expression of pancreatic carcinoma is statistically higher than that of normal pancreatic tissue.3.MVD was negatively correlative with BF and BV,along with a negative correlation between VEGF and BF. BF may be used as a imaging method to evaluate the degree of malignancy and prognosis indirectly invivo. |
