Objective:Study clinical effects of L-carnitine in treatment of community-acquired pneumonia in elderly, To enrich the treatments, and to provide clinical basis for the effects of anti-inflammatory therapy with L-carnitine.Methods:Adopt the method of randomized controlled studies, Chosen patients who visit emergency medicine department in our hospital for treatment, from June to December in2013, and meet inclusion criteria and exclusion criteria. A total of42aged patients with CAP were enrolled in this study. Use random number table, divided them into two groups randomly,24cases in normal group and18cases in experimental group. Both of them were given common treatment,including anti-infection, reducing phlegm, relieving spasm and preventing asthma,except that2g L-carnitine was given to the experimental group every day, changes of White blood cells, neutrophile granulocyte, albumin, procalcitonin and c-reactive protein were observed every three days. Results:①Patients in normal group and experimental group, there was no statistical difference in gender, average age, APACHEII score(P>0.05).②After three days’treatment, significance decreasing of White blood cells and neutrophil count were observed in two groups, first day compared with the forth day, difference was statistically significant (t=-2.459, p<0.05; t=-2.678,p<0.05),experimental group was more obvious than normal group (P0.05).③The difference of serum albumin concentration during treatment, in Normal group compared and experimental group don’t have difference (P>0.05). Treatment on the seventh day, serum albumin concentrations in normal groups have decreased a lot than before (P<0.05). White serum albumin concentration was observed in two groups, the first day compared with the seventh day, the difference have statistical meaning(P<0.05).④Treatment on the third day, concentration of serum calcitonin original (PCT)and c-reactive protein (CRP) have significantly decreased (P<0.05) before therapy. Both of the group have obvious difference compared with before (P<0.05). Conclusion:L-carnitine adjuvant therapy of community-acquired pneumonia in elderly, can enhance effects of anti-infection, relieve inflammation and improve nutritional status with less adverse reaction.
