Objective:Retrospective study the treatment of space occupying lesion focus in theposterior fossa by microneurosurgery and direct vision neurosurgery,evaluation the use value of microneurosurgery. And investigate theperioperative treatment strategy of microneurosurgery treating spaceoccupying lesion focus in the posterior fossa.Methods:chi-square test was used to compare the microneurosurgery and the directvision neurosurgery treatment of posterior fossa all three overall tumor cuttingrate and secondary surgery rate, needed treatment of hydrocephalus followingsurgery, postoperative cerebrospinal fluid leak rate and the occipital effusion.chi-square test was used to compare the microneurosurgery and the directvision neurosurgery of acoustic neuroma tumor all cut in rates, facial nerveanatomical and functional retention, cerebellar hemisphere excision. Contrastmicroneurosurgery with the direct vision neurosurgery in the surgery of themain body in the posterior fossa meningiomas of the petroclival area of tumor resection, cranial nerve damage, cerebellar hemisphere excision, postoperativecerebrospinal fluid leakage, and rank and inspection; Fisher’s exact test wascarried out in comparing microscopy and direct vision neurosurgery treatmentof the fourth ventricle tumor of the cutting rate, postoperative rate of surgicaltreatment of hydrocephalus and postoperative cerebrospinal fluid leakage.Results:Compared with the direct vision neurosurgery,Tumors were treated bymicroneurosurgery,the total removal rate was increased, reoperation rate wasdecreased, the treatment rate of postoperative hydrocephalus was decreased,postoperative cerebrospinal fluid leakage and occipital rate were decreased.Acoustic neuroma treated by microneurosurgery, the total removal rate wasincreased, the rate of facial nerve anatomy and function of retention wasincreased, Cerebellar hemisphere resection was significantly reduced. Themain body of petroclival meningioma located in the posterior fossa weretreated by microneurosurgery, the removal rate was increased. Cranial nervedamage and cerebellar hemisphere resection and cerebrospinal fluid woundleak were decreased. Fourth ventricle neoplasm was treated bymicroneurosurgery, the the total removal rate was increased, the treatment rateof postoperative hydrocephalus were decreased, cerebrospinal fluid woundleak was decreased.Conclusions: Microneurosurgery can obviously improve the effect of the surgicaltreatment of patients with space occupying focus in posterior cranial fossa,Significantly reduce the injury of operation, under the same condition ofgood neurological function after surgery, the total removal rate and subtotalremoval rate of space occupying focus in posterior cranial fossa underwentmicroneurosurgery were increased.
