Objective Whether it is to choose simultaneous or staging bilateraltotal hip arthroplasty for patients with surgical indications of bilateral hipjoint diseases is still controversial. In view of this aspect of the dispute,therefore, the study retrospectively analyzed the patients who underwentsimultaneous or staging bilateral THA and compared the clinical efficacyand safety of these two treatments, so as to provide a reference for whetherthe simultaneous bilateral THA can be chosen.Methods To have a follow-up and retrospective analysis on patientswho have received bilateral total hip arthroplasty from January2008toJanuary2010, which is58cases (116hip joint) in all:29patients (58hips)as a one-stage procedure and29(58hips) as a two-stage procedure. Thefollow-up period lasts for2years from the operation.Compare the twogroups of patients with operating time, the amount of blood loss, theamount of blood transfusion necessary, time and the total hospital cost,postoperative leg-length, preoperative and postoperative function score,and perioperative complications.Result The two groups of patients with postoperative follow-up have no complications such as incision infection, deep vein thrombosis, artificialjoints dislocation, prosthesis looseness.The only postoperative deliriumsymptoms occurred in four patients (3during the same period, while1inthe stage period). No significant difference in operating time(one-stage:117.9±23.8min,two-stage:124.1±18.8min),the amount of bloodloss(one-stage:mean,200ml,range,160-310ml;two-stage:mean,180ml,range,120-300ml),the amount of blood transfusionnecessary(one-stage:mean,300ml,range,0-400ml;two-stage:mean,200ml,range,0-300ml),preoperative and postoperative function score,postoperative complications between one-stage group and two-stagegroup。And the hospitalization costs (one-stage:mean,9.46ten thousandyuan,range,7.0-11.3ten thousand yuan; two-stage: mean,10.53tenthousand yuan,range,8.8-11.0ten thousand yuan),the length of stay(one-stage:12.1±3.2d; two-stage:20.1±3.5d), Postoperative leg lengthdiscrepancy(one-stage:0.31±0.12cm;two-stage:0.52±0.14cm), which is ofstatistical significance.Conclusion At the patient’s physical condition allowed, perioperativemanagement of technical excellence,and the doctors surgery is mature,then the one-stage THA is feasible,and it has certain advantages in thepatients’ hospitalization expenses, length of hospital stay, postoperative leglength discrepancy compared with the two-stage THA.
