Purpose:To investigate the prognostic value of seven reproducibleand reliable scoring indices or models for cirrhosis in hepatitis B virus(HBV)-related hepatocellular carcinoma (HCC) patients after curativeresection.Patients and Methods: The prognostic roles of seven preoperativescoring indices or models for cirrhosis were examined randomly in108HBV-HCC patients selected from681archival patient records aftercurative resection between January2004and September2009.Results: Of the seven cirrhosis score systems, FIB-4index(Ⅰ <1.45/Ⅱ≥1.45and≤3.25/Ⅲ>3.25) exhibited predictive value for overallsurvival (OS) and time to recurrence (TTR) by univariate (P value forOS=0.005and TTR<0.001, respectively) and multivariate analysis (P valuefor OS=0.018and TTR=0.008, respectively), respectively. Additionally,FIB-4index was associated with some clinicopathologic variables such astotal bilirubin, albumin, creatinine, international normal ratio and tumorsize, respectively. Moreover, Metavir score (F0-1/F2-3/F4) was alsoassociated with poor overall survival (P=0.042) and high recurrence rate(P=0.012) by uni variate analysis. Furthermore, combination of Metavirscore and FIB-4index had larger area under the receiver operatingcharacteristic curve for OS (0.618,0.635and0.669, respectively) andTTR (0.654,0.670and0.708, respectively) than them alone. Importantly,in normal AFP subgroup, FIB-4index (Ⅰ/Ⅱ/Ⅲ) could discriminate the outcomes of patients with high or low OS and TTR (P=0.001and <0.001,respectively).Conclusion: Preoperative FIB-4index is a simple and inexpensiveprognostic marker for the poor prognosis of HBV-HCC after curativehepatectomy.
