AIM: To investigate whether prophylactic antibiotic therapy isffiective for patients with severe acute pancreatitis.Method: A computerized literature search of Medline, Elsevier,Embase, CNKI, WILLEY databases was conducted to identify relevantarticles published from January1990to May2013. According to theinclusion criteria,12studies were selected. The data were analyzed usingRevMan5.0software.Result: We pooled718patients from12randomized controlled trials.There were356patients with SAP who received prophylactic antibiotics,and362patients were randomized to the placebo arm. Our meta-analysissuggests that prophylactic antibiotic treatment did not significantly reducemortality (RR0.78,95%CI0.53~1.15, P=0.20) or the incidence ofinfected pancreatic necrosis (RR0.77,95%CI0.58-1.02, P=0.07),nonpancreatic infections (RR0.72,95%CI0.49~1.06,P=0.09), orsurgical intervention (RR0.90,95%CI0.67-1.19,P=0.45).Conclusion: Prophylactic antibiotic treatment does not reducemortality in patients with severe acute pancreatitis and has no significantprotective effect in patients with SAP.
