Research On The Fatigue Status And The Relevant Factors Of Patients With Chronic Heart Failure

Objective:(1) to describe the state of fatigue in patients with chronic heart failure;(2) to understand the clinical features of disease, depression, anxiety, quality of life, nutrition and sleep quality in patients with chronic heart failure;(3) to remember the factors related to the state of fatigue in patients with chronic heart failure and the influence level, and provide a theoretical basis for intervention in the treatment of chronic heart failure patient fatigue.Methods:From2013September to December, a cross-sectional survey research method in epidemiology, comply with the uniform inclusion and exclusion criteria, was randomly sampled in Changsha City3three level of first-class hospital in258patients with chronic heart failure as investigation object. The research tools include self-made general questionnaire, multi dimension Fatigue Scale (MFI-10), the hospital anxiety and Depression Scale (HAQ), heart failure quality of life scale (LHFQ), Mini Nutritional Assessment Scale (MNA) and Pittsburgh sleep quality index (PSQI). All data were processed using statistical software SPSS16.0, statistical methods including descriptive statistics, two independent samples t test, F test, Spearman correlation analysis, multiple stepwise regression analysis.Results:1)258patients with chronic heart failure incidence was97.51%, the total average fatigue (67.73±12.96), the5dimensions of general fatigue, physical fatigue, reduced activity, decreased motivation and mental fatigue scores were (14.93±3.69),(17.09±3.21),(15.01±3.50),(10.66±2.87),(10.03±4.36).2) On the anxiety in patients with chronic heart failure and depression level survey results showed that:63.74%of the patients or suspected anxiety symptoms,67.44%of the patients had symptoms of depression or suspected presence.3)258cases of total score of life quality in patients with chronic heart failure was (73.28±19.43)%, suggesting that the poor quality of life of patients, the physical domain score was (30.98±8.72),(15.28±emotional domain score is4.72), other areas score was (27.02±8.07).4) Nutritional status survey:258of patients with MNA scores were (18.02±4.51), which nourished patients for21people, accounted for8.14%, there is the risk of malnutrition patients accounted for55.81%to144people, there were93, undernutrition, accounting for34.05%of the total number, patients generally there are cases of malnutrition.5)258patients with chronic heart failure PSQI is divided into a total of (10.49±4.37), which exist in the sleep disorders in patients with195people (75.58%).6) Different gender, occupation, medical convenience degree, self-rated health, heart failure etiology (primary disease), factors of heart function classification, anemia, anxiety, depression, nutrition, sleep quality, quality of life in patients with chronic heart failure fatigue score or scores correlated (P<0.05).7) Multiple regression results show:the total score of life quality, the scores of MNA, daytime dysfunction, physical domain scores, subjective evaluation, dietary assessment, depression, sleep disorders, emotional domain score is the impact of the overall fatigue in patients with chronic heart failure and the dimensions of the main factors (P<0.05).Conclusion:1, Widespread fatigue in patients with chronic heart failure, mainly for the physical fatigue.2, Chronic heart failure patients generally have anxiety and depression, sleep disorders, malnutrition, poor quality of life.3, There is a positive correlation of fatigue status of patients with chronic heart failure and depression and anxiety, sleep quality score, there exists a negative correlation with the nutritional status and quality of life in patients with.4, Effects of chronic heart failure patients fatigue is the main factor scores of quality of life, MNA score, daytime dysfunction, anemia, whether physical domain scores, subjective evaluation, self-rated health, living convenience, dietary assessment, depression, sleep disorders, emotional domain score.
