| Background:AIDS is a serious public health problem that is severely affecting people’s health. AIDS antiretroviral therapy has gotten some achievements with the promoting of HAART. But there is no effective way to eradicate HIV so far. In recent years, the role of cytokines in the pathogenesis of AIDS and antiviral therapy has aroused increasing attention. Scientists have discovered a variety of cytokines involved in anti-HIV immune response and immune reconstitution, and put forward that some cytokines could be adjuvant therapy methods to strengthen the effect of anti-HIV therapy. IL-27is a new cytokine in the field of HIV research. There are in-vitro experiments found that IL-27may possess some therapeutic value in HIV-1viral infection, but IL-27-related clinical research is less to now. This study will examine the expression of IL-27and its receptor in HIV infection, and explore its role in the pathogenesis of HIV/AIDS and HAART anti-viral immune response.Objective:To research the expression of IL-27and its receptor in HIV infection and their difference between the HIV patients and the healthy; the impact of HAART on the expression of IL-27and its receptor. To analyze the relationship of IL-27and its receptor gp130with CD4+T cell count, HIV-RNA viral load, the relation between IL-27and IFN-γ which is typical Thl cytokine, and the relation between IL-27and IL-4which is typical Th2cytokine. To explore further the pathogenesis of AIDS and immune reconstitution mechanism of anti-viral therapy. Methods:Selected34HIV-positive cases who are the Second Xiangya Hospital AIDS specialist outpatient from December2012to March2013and they were ready to start HAART, and followed up regularly to collect their blood samples when they before HAART(0month), already started HAART6months and12months. Meanwhile, we randomly selected27health cases as a healthy control group. Used enzyme-linked immunosorbent assay (ELISA) to test the concentration of IL-27, IFN-γ and IL-4in plasma, and used flow cytometry to test the CD3+CD4+T cell count, the expression of gp130on the surface of CD4+cell in peripheral blood mononuclear cells (PBMC). The peripheral blood HIV viral load were tested by HIV real time PCR, and the expression of gp130mRNA were tested by RT-PCR and agarose gel electrophoresis. Then we analyzed all the data with statistical method.Results:The IL-27average plasma concentrations of untreated HIV-positive cases were significantly lower than the healthy control group. IL-27concentration increased gradually after HAART, as the concentration of IL-27in cases who started HAART6months and12months was significantly higher than in untreated cases. The untreated HIV-positive cases expression of gp130on the surface of CD4+cells in PBMC was significantly lower than the healthy control group. This situation would improving with HAART, the expression of gp130in cases who started HAART6months and12months was significantly higher than in untreated cases. This trend was confirmed by detection the gp130mRNA expression with RT-PCR and agarose gel electrophoresis. IL-27and gp130were positive correlated. IL-27and gp130were positively associated with CD4+T cell count, negatively associated with HIV viral load. The plasma concentration of IFN-γ in HIV-positive cases was increasing with HAART when the concentration of IL-4was decresing, but they are both close to the normal. The change trend in the concentration of IL-27and IFN-y are similar, but the trend is opposite between IL-27and IL-4.Conclusion:1. We first observed the IL-27plasma concentrations of HIV-positive person were lower than the healthy, and the concentration gradually increased with HAART. Similarly, the expression of gp130which is the receptor of IL-27in HIV-positive person was significantly lower than the healthy. But with long-term HAART, this damaged receptor expression can be gradually restored.2. We found that the concentration of IL-27and the expression of gp130were negatively correlated with HIV viral load and positively correlated with CD3+CD4+T cell count in HIV-positive person. Suggested that IL-27may have a protective effect to HIV infected person, it might be involved in the pathogenesis of HIV and anti-viral immune response mechanism.3. There must be certain relationship between the concentration change of IL-27and IFN-γ/IL-4.Suggested that the pro-inflammatory of IL-27is dominant in HIV/AIDS, and that maybe associate with the imbalance of Thl/Th2cytokines. |
