Clinical Relevance Of EGFR In Patients With Adult Idiopathic FSGS

Objective:To investigate the patients with focal segmental glomerulosclerosis (FSGS) to analysis the estimated glomerular filtration rate (eGFR) stage and related indicators. Methods:81cases of focal segmental glomerulosclerosis, were divided into3groups according to the eGFR staging of chronic renal disease:chronic kidney disease Stage1eGFR>90ml/min. chronic kidney disease stage2:90ml/min>eGFR>60ml/min chronic kidney disease stage3:60ml/min>eGFR>30ml/min. blood pressure, creatinine, urea nitrogen, cystatin, hemoglobin, uric acid, urinary albumin, serum cystatin PTH, serum calcium, phosphorus, cholesterol, triglyceride, and other related factorsin in each different stage of eGFR between the groups and groups of related index are different from the mean of the Xinjiang normal person. Results:the perpon of FSGS occurs more often in young people, age (38.91±11.964) years old, male to female ratio:1.07:1,82.72%patients with urinary protein, the massive proteinuria (24hour urinary protein quantitative>3.5g/1) accounted for27.10%, hematuria (red blood cell count>10000/ml) accounted for80.25%,49.38%of FSGS patients complicated with hypertension,The FSGS patients with large amount of proteinuria, hematuria, hypertension in eGFR stage was statistically significant (P<0.05),with the progression of eGFR, uric acid, creatinine,cystatin, urea nitrogen are increased (P<0.05), hemoglobin are decreased (P<0.05),the FSGS patients with body mass index, cholesterol, parathyroid hormone, serum calcium, serum phosphorus, serum albumin, serum total protein in eGFRl-3stage had no statistical significance (P>0.05). The value of Cystatin, microalbuminuria is different with Xinjiang normal people in the average value of eGFR1(P<0.05), The value of uric acid, urea nitrogen, creatinine are different with the average value of normal people in Xinjiang in eGFR2and are different (P<0.05), the value of triglyceride, hemoglobin, serum calcium, parathyroid hormone at stage3are different with mean values in Xinjiang normal people(P<0.05). The hemoglobin, creatinine, uric acid is an independent risk factor in FSGS from stage eGFRl to2(p=0.018,0.003,0.031). The other value of the indexes were not statistically significant. Conclusion:primary FSGS occur in young, male, insidious onset; the most common clinical is proteinuria, second is hematuria, FSGS patients with hypertension, hematuria, proteinuria are more faster in eGFRl to3, early reduction of creatinine, urea nitrogen, uric acid occurs during the eGFR2period, hemoglobin, triglyceride, serum calcium, parathyroid hormone abnormalities occur in phase eGFR3, urinary albumin, serum cystatin appear abnormal changes in eGFRl period, the indexes abovly, we can study metabolic complications and time of each phase of eGFRon patients with FSGS, this disease can be targeted for treatment, early diagnosed, early treatmented, clearly understand the risk factors accelerates the progress of the FSGS patients with eGFR, and make forward constructive suggestions about prognosis of the predictions, and the formulation plan of the treatment.
