Objective:To establish beagle animal model with intercostals nerve,using the rectus peritoneal flap to repair the tongue defect, anastomose the intercostals nerve with hypoglossal nerve, observing by whether use the ciliary neurotrophic factor (CNTF) or not to evaluate the quantity of regenerated nerve and the difference of functional recovery, providing experimental evidence for functional tongue reconstruction. Methods:Use18healthy male beagle dogs, aging between1and1.5years, numbered them respectively and divided them into two groups (group A, B) according to random number table, each group has nine dogs. First, the two groups on the left tongue defects produced in the left submandibular first molar to the vertical center line of the front of the tongue to connect bounded4cm×3cm wide excision of tongue tissue, and then each makes preparation of the11th intercostals nerve and the rectus-peritoneal flap with abdominal arteries and veins, to anastomose the11th intercostals nerve with hypoglossal nerve, anastomose the abdominal arteries and veins with lingual arteries and outside the jaw vein respectively two groups’match period are put in the silicone tube, closed the both ends of it by collagen.After anastomosis, injection the ciliary cell nutrition factor300ng (5ul,60ng/ul) one-time into the group A silicone tube, injection5ul saline solution into group B silicone tube.12weeks after surgery, two groups of animal model make hypoglossal nerve electrophysiology detection, to observe regenerated myelinated nerve fibers, and motor fiber count, and measure the nerve diameter and the target muscle myocyte diameter and its area. Results:Two groups of animal model make hypoglossal nerve electrophysiology detection, the incubation period of A, B two group was obviously longer than normal, comparing the two groups, group A’s incubation period is shorter than group B;A, B two groups’amplitude is reduced than normal, comparing the two groups, group A amplitude is greater than group B;A, B two groups’conduction velocity is slower than normal, comparing the two groups, group A’s conduction velocity is quicker than group B, all the differences were statistically significant (P<0.05);A, B groups’regenerated nerve diameter, myelinated nerve fibers, Motor nerve fibers and fiber counts, group A is greater than group B, and all differences were statistically significant (P<0.05); Cells’diameter and area of transplant rectus, group A is bigger than group B, and the differences were statistically significant (P<0.05). Conclusion:After using the rectus peritoneal flap carrying intercostals nerve transfer to repair the tongue defect, both two model groups’intercostals nerve can regain the control of hypoglossal nerve, and application of CNTF can promote regeneration of motor nerve, hypoglossal nerve functional recovery is better than that of unused groups, and can effectively prevent the target muscle atrophy.
