Objective:To study the core decompression treatment early avascular necrosis of the clinical effect and imaging changes. Methods:In the orthopedic Sixth affiliated hospital of XinJiang medical university admitted from Jan,2008to Jul2005diagnosed with International circulation research (ARCO) the oseteonecrosis of the femoral head staging criteria:ARCO stage Ⅰ~Ⅱ patients,67patients (99hips),treatment group (40cases (57hips), femoral head pulp core decompression treatment; Control group of27cases (42hip), drugs, and comprehensive treatment. Before and after treatment1, march to Harris score, June review postoperative pelvic plain film, double hip side slice, MRI and Harris hip score. Using SPSS17.0software for statistical analysis. Results:After5years of follow-up, the treatment group (57cases of hip, eventually28underwent hip replacements, accounted for49%; The control group of42cases of hip, of which29routine joint replacement, accounted for69%, eventually hip replacement rate of treatment group was obviously lower than the control group; Patients with medullary hip core decompression significantly delayed the joint replacement time, P<0.05, differences between the two groups have statistical significance. Control group hip replacement rate peak for2to3years postoperatively, the treatment group hip replacement rate peak for3to4years after surgery. Treatment group patients with medullary core decompression preoperative and postoperative Harris score and necrosis index, according to the comparison of P<0.05, differences between the two groups was statistically significant, effective treatment; Patients with preoperative Harris score and necrosis index (p>0.05), no obvious correlation after line pulp core decompression surgery and follow-up, patients with Harris score and necrosis index is still no significant correlation (p>0.05), explain Harris score and necrosis index has no obvious correlation, and after marrow core decompression surgical intervention, remains unchanged both no correlation.Conclusion: Bone marrow core decompression is a classic for early treatment of ischemic necrosis of femoral head surgery treatment. To ease the pain symptoms, improve the walking ability and improve the limb function has the obvious effect, delay the time of total hip replacement. Especially suitable for ARCO Ⅰ~Ⅱ options were bone necrosis of the patients.
