Objective: To identify the incidence, characteristics, etiology andrisk factors of pulmonary infection after allogeneic hematopoietic stemcell transplantation (allo-HSCT) recipients and raise physicians’awareness and diagnostic accurancy of it, reduce the diagnostic errors atthe same time. Methods: The clinical date of37allo-HSCT patients treated at ourtransplantation unit between January2010and December2012wereenrolled into this retrospective analysis. Results: The cumulative incidences of pulmonary infection afterallo-HSCT is45.9%(17/37). And33episodes of pulmonary infectionwere recorded in the17patients. Among them,11patients hadreduplicative episodes of pulmonary infection, but only39.4%(13/33)of them were microbiologically defned pulmonary infection. Fungipredominated during the post-engraftment period especially thecandidiasis. By univariate analysis, human leucocyte antigen (HLA)disparity and graft versus host disease (GVHD) were closely associated with the occurrence of pulmonary infection. By multivariate analysis,HLA disparity and GVHD were significantly associated with theoccurrence of pulmonary infection (P<0.05).Conclusion: Pulmonary infection is still common followingallo-HSCT, which has contributed to transplant-related morbidity andmortality strikingly, and threatened these patients greatly. HLA disparity,GVHD made the significant risk factors for pulmonary infection afterHSCT.
