Objectives: To systematic review the effectiveness of transitional carefor patients with Alzheimer’s Disease in China; to analyze the gaps ofChinese transitional care for patients with Alzheimer’s Disease inorganization and implementation,and to provide theoretical evidence forstandardized transtional care mode for patients with Alzheimer’s Disease inChina.Methods: According to the standard provided by Cochrane Handbookfor Systematic Reviews of Interventions, published literatures weresearched through Cochrane Library, PubMed, ScienceDirec, ChineseBiomedical Literature Database(CBM), CNKI, VIP, Wan Fang database.These databases were searched to collect the randomized controlledtrials(RCTs) of transitional care for patients with Alzheimer’s Disease inmainland China published in Chinese or English. The retrieval time waslimited to November,2013. All the literatures were selected according to thestrictly designed strategies.The Cochrane Handbook was uesd to evaluatethe quality of the literatures. Using of qualitative analysis to describe the characteristics of the literatures.All data were extraccted using a standardform and RevMan5.1software was used to analyse the extracted data.Results:A total of12randomized controlled trials of145initially articles metinclusion and exclusion criteria after a thorough literature seaarch.Results of evaluation showed method quality of inclued studies wereGradeB. The large sample size was120and there were a total of803participates in this review.The effectiveness of transitional care for patients with Alzheimer’sDisease:MMSE: Weighted Mean Difference,WMD=2.12,95%ConfidenceInterval,CI (1.59,2.65), p<0.000001, the difference is statisticallysignificant. ADL: WMD=0.13,95%CI(-5.31,5.57),p=0.96, there isno difference between two groups. SF-36: WMD=162.13,95%CI(128.46,195.79),p<0.00001, the difference is statistically significant.Conclusions:1Transitional care for patients with Alzheimer’s Disease has beenpaid attention in mainland China.The number of related studies increasdobviously since2010.2Transitional care can improve cognitive function and quality of lifeof patients with Alzheimer’s Disease, in order to reduce the burden onfamilies and society in China. 3All inclued studies were with middle risk of bias and the samplesize was small with weak representation.4Transitional care for patients with AD is a new nursing modelwhich can satisfy the actual needs of.the patients and their relatives.Develop our transitional care model for patients with AD should fullyutilize the current positive conditions and conjunct with the actual situation.
