Clinical Analysis Of The Surgical Treatment Of Pulmonary Artery Sling

Objectives: Summarize the clinical experience surgical treatment ofcongenital pulmonary artery sling.Methods: Collect16cases of PAS which had the treatment of operationin our hospital during the period from September2009to February2014,10males,6females, age range2.5month to42month(average13.2±11.2m),weight range4.5kilogram to15kilogram(average8.3±2.5Kg).All the patients have had examined by CT and CTA andEchocardiography,10of them associated with intracardiac anomalies, CTairway remodeling indicate that all the patients exist airwaystenosis,degree from29%to63%,range from11%to49%.All the patientshave had the operation of pulmonary artery sling total correction under thecardiopulmonary bypass, one have had an operation of slide tracheoplastyat the same time, one have had an operation of tracheal resection andpatch enlargement again after the correction.Results: The average breathing machine time after the operation199.3±376.0hours, average hospital time after the operation17.3±12.2 days. All the patients have had examined by CT and Echocardiography, allpoint out anastomotic blood flowing. There are6patients have died, themortality rate of37.5%, the other patients leaved the hospital,4of them lostto follow-up, the other6patients were followed up from1month to4years,there respiratory symptoms and growth conditions were improved varyingdegrees.Conclusion: Children with recurrent respiratory tract infections should beconsidered with pulmonary artery sling clinically, when the disease wasdiagnosed, surgical treatment should be considered. The age is smaller, thepostoperative respirator use is longer, the risk of death is higher. Thetreatment of combination with tracheal stenosis need to be careful andconsider staging surgery when necessary.
