Objectives: In this experiment, we aimed to observe the effect on the8-iso-PGF2αthrough Perindopril of different doses to cure the patients with chronic heart failure,which viewed the serum NT-proBNP as a reference. We intended to explore thecorrelation of8-iso-PGF2α and left ventricular remodeling in the patients withchronic heart failure correlated, which revealed the role of8-iso-PGF2α in thepathogenesis of heart failure and clearly provided a theoretical basis for thepathogenesis of heart failure. And it could also prove the possibilities that Perindopriltreat the patients of heart failure though the8-iso-PGF2α way and the feasibility andsafety of the treatment of high dose ACEI.Methods: In this experiment, we chose68CHF patients and42healthy persons asresearch subjects. The CHF patients were given by the treatment of anti-heart failurewith digitalis, diuretics, vasodilator and small doses of β-blockers after admission. Onthis basis, the patients in smaller dose group were treated with Perindopril of4mg/dwhile the patients in larger dose group were treated with Perindopril of8mg/d foreight weeks. The3ml blood samples of the antecubital vein were collected from thehealthy individuals and CHF patients. We separated the plasma by EDTA-Na2anticoagulant at4℃to2500rev/min with a high-speed desktop centrifuge for10minutes and preserved in the refrigerator with-80℃. All the specimens weremeasured by ELISA to detect the serum concentrations of NT-proBNP and8-iso-PGF2α. We used the Color Doppler ultrasound to measure the left ventricularsystolic function including left ventricular ejection fraction, left ventricular systolicdiameter and left ventricular end diastolic diameter. After8weeks, the CHF patientswere collected the blood again to detect the serum concentration of NT-proBNP and8-iso-PGF2α. All the statistical data were analyzed by the SPSS13.0software.Result:1. The serum levels of NT-proBNP and8-iso-PGF2α in the patients of the CHF group were significantly higher than the those in the patients of the controlgroup (P<0.001);2. The serum levels of NT-proBNP and8-iso-PGF2αincreased bythe severity of heart failure, and the differences between the groups were significant(P <0.001);3. The correlation between the serum levels of8-iso-PGF2αandNT-proBNP in the patients with CHF were positive before and after thetreatment(P<0.001);4. The serum levels of NT-proBNP and8-iso-PGF2αin largerdoses dropped significantly higher than the smaller dose group (P <0.05);5Thedecline of the LVEDD, LVESD and LVEF in the patients between the large dosegroup and the small dose group had no significant difference(P>0.05).Conclusions:1.8-iso-PGF2α had high specificity and sensitivity in the diagnosis ofCHF as well as to determine the efficacy. And8-iso-PGF2α could be used as areference to reflect the degree of the ventricular remodeling;2. Perindopril couldreduce the serum level of8-iso-PGF2α in patients with CHF. And it was possible thatPerindopril could improve the myocardial remodeling by the way of8-iso-PGF2α andeffect on the RAAS system to treat heart failure;3. The larger doses of perindoprilcould significantly reduce the serum levels of8-iso-PGF2α in patients with CHF.
