| Objective: To discuss the effects of clinical nursing pathway(CNP) onprevention of lower deep venous thrombosis (LDVT) in patientsundergoing radical hysterectomy and to establish the best nursing mode,reducing the risk and incidence of LDVT in patients, the length of hospitalstay, the hospitalization expenses and improving the quality of nursing careand patients satisfaction.At the same time,to investigate the dynamicmonitoring of plasma fibrinogen,D-dimer and platelet of LDVT inpatients undergoing radical hysterectomy on early diagnostic value andclinical significance.Methods: This paper is a retrospective study to discuss the effects ofCNP implementation.They are divided into experimental group and controlgroup. The control group choose the radical hysterectomy patients90casesfrom August2011to June2012,which the patients were cared under thetraditional nursing ways and the experimental group choose the radicalhysterectomy patients122cases from August2012to June2013, which thepatients were cared according to the CNP as soon as hospitalization.Record different index in the two groups, including length and spent of hospital stay,disease related knowledge and patients satisfaction.Dynamic monitoring plasma fibrinogen,D-dimer and platelet of theexperimental group.Results:1. No statistic difference was observed in the age,culturelevel, way of payment, professional and operation time between theexperimental group and control group(P>0.05).2. There was statisticdifference in the length of hospital stay and hospitalization expensesbetween the experimental group and control group(P<0.05).3. Theexperimental group had a better score for disease related knowledge andpatients satisfaction than that of the control group(P<0.05).4. There wasstatistic difference in the incidence rate of LDVT between the experimentalgroup and control group,with the experimental group occurred in2casesand8cases in the control group(P<0.05).5. The different level of plasmafibrinogen,D-dimer and platelet during the phases preoperative and2dayspostoperative in NO LDVT group was not significant (P>0.05).6. Thedifferent level of plasma fibrinogen and D-dimer during the phasespreoperative and2days postoperative in LDVT group was significant (P<0.05).7. The different level of plasma fibrinogen, D-dimer and plateletbetween two groups: preoperative was not significant (P>0.05),2dayspostoperative was significant in fibrinogen and D-dimer(P<0.05).,but notsignificant in PLT(P>0.05).8.The dynamic change of plasma fibrinogen,D-dimer and platelet in the experimental group has clinical significance.Conclusions:1. CNP can effectively reduce the incidence rate ofLDVT in patients undergoing radical hysterectomy.2.By optimizing thenursing process through CNP, we can ultimately achieve the goals of reducing the length of hospital stay,improving the level of diseasecognition and patients satisfaction.3. Determination of plasma fibrinogenand D-dimer has early diagnostic value.4.
