Aim:Assess the effect of vitamin D supplementation on glycemicstatus, insulin resistance, insulin secretion and diabetic complications.Methods:Systematic search English-language studies published priorto31October2013via MEDLINE, EMBASE and Cochrane Library. Ameta-analysis of randomized controlled trials (RCTs) comparing vitamin Dor analogs with control in parameters of glucose metabolism wereperformed. Study selection and meta-analysis were conducted according tothe quantitative Jadad score system combined with the Cochrane Handbookfor systematic reviews. Data extraction and study quality assessment wereperformed independently by two reviewers and analyzed by RevMan5.0software.Results:21articles (n=2712) were included in the systematic review.Compared with control group, vitamin D supplementation provided astatistically significant increase in insulin secretion (effect size0.23,95%CI 0.04-0.41, P=0.02). And the effect seemed more significant in prediabetesand obesity subjects (effect size0.42,95%CI0.12-0.71, P=0.006; effectsize0.32,95%CI0.05-0.59, P=0.02; respectively). No significant decreasewas seen in fasting glucose, HbA1c or insulin resistance. In the subgroupanalysis,7RCTs with normal glucose tolerance at baseline showed a smallbut significant increase in fasting glucose in the vitamin D group comparedwith placebo (effect size0.14,95%CI0.01-0.28, P=0.04). Currently,studies on effects of vitamin D supplementation on diabetes complications(diabetic neuropathy, micro-and macro-vascular outcomes) wereinsufficient to draw conclusions.Conclusions: Find weak evidence to recommend vitamin Dreplacement for improving glycaemic outcomes and complications ofdiabetes. And it deserves further investigation.
