Objective:To investigate the effect of MDR1and SLCO1B3gene polymorphisms on postoperative tacrolimus requirements in renal transplant recipients.Methods:Patients who were scheduled to undergone renal transplantation in the Third Xiangya Hospital of Central South University were enrolled in this study, male or female, aged18to65years, the whole blood concentrations of tacrolimus and tacrolimus concentration/dose (C/D ratio) were detected by EMIT during3-6months after surgery. All Patients received tacrolimus+mycophenolate mofetil+corticosteroids immunosuppressive therapy after renal transplantation, MDR13435C>T, SLCO1B3699G>A were genotyped by PCR-RFLP. The differences in tacrolimus C/D ratios were compared among their genotypes.Results:There were no significant difference in tacrolimus C/D ratios among the genetic polymorphisms of MDR13435C>T (P>0.05); SLCO1B3699G>A genetic polymorphisms showed significant difference in tacrolimus C/D ratios (P<0.05).Conclusions:SLCO1B3699G>A polymorphisms is the factor contributing to individual differences in tacrolimus therapy, patients who carried AA genotype were found to have increasingly higher tacrolimus C/D ratios.
