Objective: Knee osteoarthritis (KOA) is usually diagnosed bases onclinical manifestations and imaging examination.But this two methods for earlydiagnosis of KOA has no specificity and sensitivity. When Knee osteoarthritisdiagnosed by symptoms and radiographic manifestations.The disease hasdeveloped to a considerable stage. bringing patients and their families asubstantial economic burden. The metabolites articular cartilage type II collagencan quantitatively and dynamically reflect the changes in the articular cartilageThis study to observe the relationship betweem the C2C level in urine andseverity of knee osteoarthritis(KOA).and to further explore osteoarthritis of theknee cartilage metabolism pathology Methods: In this study the subjectsincluded KOA patients after the menopausal liver function abnormalities noother diseases affecting cartilage metabolism not taking drugs of hormones andcartilage metabolism.The control group (non-KOA group) from the same agegroups not knee osteoarthritis symptoms and radiographic changes. Ninetypatients with KOA were taken as KOA group. Twenty healthy volunteers wereselected as control group. Moming urine5mL and antero-posterir and lateralposition X rays of knee joint were collected from both groups.Moming udninesamples were stored at-80℃.Findings on their images were classed according to the Kellgren-Lawrence radiographic grading criteria. Divided into fivegroups:control group group I group II group III and group IV. Control groupincludes20cases8males and12females; group I includes31cases,11malesand20females; group II includes14cases6males and8females; group IIIincludes23cases8males and15females; group IV includes22cases4malesand18females.There were no significant differences in age and sex of eachgroup.C2C level in urine was detected by Enzyme-linked immunosorbent assay(ELISA).Results: C2C level in KOA group(218.341±22.270pg/mL) wassignificantly higher than that in control group(261.235±39.944pg/mL)(P<0.01). Comparision of C2C level in each group: group IV(311.872±18.759pg/mL)>group III(273.753±18.595pg/mL)>group II>(256.251±23.379pg/mL)group (I219.009±13.431pg/mL)>or control group(218.341±22.270pg/mL)(P=0.000).urine levle of C2C in group I and control group were similar,there was no significant difference (P>0.05). Conclusions: Early KOA patientshave no significant changes in the level of C2C The content of urinary C2C stepup in knee osteoarthdtis patients.And urinary C2C has direct correlation withknee osteoarthdtis.So C2C can be a marker to distinguish severity of KOA canalso be used as a reference index to the evaluation of drug efficacy and surgicaltreatment of KOA.
